Does anyone else feel the assignment of error scores are excessive?  Do you get major errors for things that should be minor?  So disheartening.

I also feel that they absolutely lambaste you for things that shouldn't be. I take great care with my reports, when i check them in QA Feedback they look almost perfect, except for blanks.

When I get an Audit back, it is RARELY that I get over about 98-99.0%. I have been written up for this a lot, and the funny thing is, up until about 8 months ago, NO ONE EVER KNOCKED MY WORK! I was always told it was about perfect, no problems!

Now with these new standards, if you have every one word off, you get slammed for it!

I got a Critical Error mark for typing:

AK-7692 for an EEG Number instead of
NK-7692 or something.

Is that so terrible??

Some of the things they claim I typed, it almost looked like they planted it and falsified my work.

That is what they have done, "upgraded" the level of every error so they can deduct more from your score, not to mention nitpicking. How else to explain when I was at Nuance my scores were below 99% and now at my new job they are 99.8%? Simple, Nuance has the system rigged to screw you... and as far as plants, I think so too.

Sorry, but your typos are not that crucial. You are NOT the last line of defense against certain death. This is especially true with electronic records.

The reports you type are not the only sources of information in the patient's record. They are primarily summary reports that duplicate information found elsewhere. That is why there are big pauses in the dictation--the doctors are reading the original information in the record.

They are useful for speed and convenience, but policies generally prevent acting on anything in them. Drugs? See pharm and orders. Surgery? See the surgery documents.

The general quality of transcribed documents is terrible. As a former MT, I am shocked by what I see coming from "99+%" MTSOs. Maybe your work is great, but the rest of it is rot. You have no idea.

Further, you cannot know if the dictator is dictating something wrong. Maybe you catch right or left and male vs female, but those are utterly inconsequential.. You can't tell if he mixed two people up.

This whole life and death thing was cooked up by AHDI in an attempt to professionalize the job and raise the status and pay. The only people who bought it were MTs.

There IS a role for quality assurance with regards to medical records, but it does not involve focusing on typos. That is a job you could retrain for.

Yes, we should try to always do the very best job we can and aim for 100% every single day, every single job but always keeping in mind that what we do is more important to the hospital billing office and the coders than to actual patient care. We do a job, but we are not all that!

This is pretty much what I told my super when I was being raked over the coals for not being able to find a dosage for a med or frequency of dosage. We are told we're supposed to either know it or find it. We are told if we can't understand it to blank it, then berated for blanks. The reports most of us do are NOT the first report most docs grab. There are many other specific reports concerning any patient. Thank you for being able to articulate what we knew.

JC already has standards that cover what is important to patient care.

Their scope does not cover labor disputes. They already declined to pursue this angle. You are wasting your efforts and their time.

Please stop making MTs look foolish because they do not know this.

I did, trying to defuse some of the hysteria about errors being "critical"... MAJOR ERROR ALERT... and so forth, the exaggeration of errors to make us fearful and of course, to keep us from making bonuses.

Once again, these mistakes do not originate with us, but we are the ones who are supposed to catch them! IF that is so, and our job is THAT important, THEN PAY US WHAT WE ARE WORTH! Yesterday, I had a report that say that labs were done on the 14th and the BUN was....blah blah. It printed out BUN 14, completely ignoring the rest of the sentence! If I let that go through? ARE JOBS ARE SOOOOO IMPORTANT....PEOPLE'S LIVES DEPEND ON THEM....(How many times have you heard that?) THEN PAY US WHAT WE ARE WORTH! Treat us with respect, not like we are expendable, and THEN we will give you our BEST.

This mistake DID originate with the MT, She admitted making - error, but called it a typo. nm

if they review at all of our work but because most are 100 percent, they don't upload to Fiesa, just choose ones with 99 percent or below and occasionally throw in one that is 100 percent to make it look good!!  I do NOT trust anythng they do and would not put it past them. We have no way of listening to even make sure that what they audit us on is accurate.  I am so paranoid that I think they may even change things that we type to make it an error!  So disgusted..... ...

Seriously, the only person I trusted for corrections was my QC.  If they are not going to be doing the audits, who is?  MMEs are just upper grade MTs who were suckered into working for less pay.  MModal has fired/laid off/chased off most of their competent MTs.   I can hardly wait until I get my first (and probably last) required audit.  A 99.6% accuracy requirement from this company of corporate incompetents is hysterically funny. ...

Late last week I recieved an email communication about how they will only audit reports that were transcribed/edited during the current work week, only to log on this morning (I'm off Fri and Sat, so Sun morning is the 1st day of my week) to find that I had a post audit score below 99% that was audited on Saturday, so obviously it was edited last week sometime.  If I don't start my week until Sunday, how on earth can they audit a report on Saturday that was submitted the week prio ...
