Oh how I want to STRANGLE this dictator... - Is it too much to ask to

spell the name of a doctor whose name is something like "Dr.  Lhaksuduaramamaan" -

He just says it real fast and thinks whoever is transcribing the report will just automatically know how to spell that!!!  Not to mention he has a Chinese accent --THAT doesn't help either. So he can't go ahead and maybe spell that out?  To top it off, he says this name 3 different times in the report, making me have to leave THREE blanks, which I now have to pend?  I was miraculously able to figure out the rest of his jibber jabber so if not for THIS I wouldn't have to pend.  Oh, and it's a super long report, too.  There goes my QC % for the week.  Thanks a lot DOC !!!! 

Does anyone else find themselves getting ENRAGED by stupid dictators?  It's not even entirely their fault (it's mostly NUANCE's), but I still get so MAD at them !  I'm ashamed to say what I wish would happen to them when they make my job IMPOSSIBLE and also basically make me lose income !

Yes, I know this is the least of the problems of this "career" anymore, but still.  UGH!

I sent this to editing (precisely this):

Obviously I could not make out blank 1, and blank two was because of an unfinished sentence.

So, ladies and gentlemen, this is the feedback (editing) I got:

Median, radial, ulnar and PIN are intact. Motor to the upper extremity.

No added word, no finished thought, just a period! And someone wants me to "sign off" on this garbage?

The absolute best part? This document went to the client like this still bearing MY initials, not those of the ever-so-helpful editor.

Flippin' ludicrous.

I'll never forget working in a hospital when the hospital was undergoing remodeling. The doctors were actually dictating with jackhammers going on in the background! Now really don't they have any OTHER place to dictate that's quiet in a hospital for crying out loud!? You'd think, right? My favorite though was when one of my doctor was dictating while he was in the bathroom!!! If he knew that I could hear EVERYTHING going on, I bet he might've thought twice... I think... who knows anymore... LOL. JOS

We can only hope that the clients will see through this charade before long. If I were in charge of finances at the hospital, I would withhold payment for crap like this.

Is the whole purpose just to slow me down enough to be fire-able? I'm the one who has been seeing whacked out stuff on reports a lot lately, like, "This is a normal low-voltage fast activity EEG."

"...borderline left ventricular hypertrophy" and then going on to say "left heart size normal" in practically the next paragraph.

From doctors I have been transcribing for years. At the rate I am having to Google every single little phrase uttered, I am probably averaging maybe 100 lines per hour. I do not check line counts usually because there has never been an issue, but this? This is really wierd stuff! And it is not just one or two "off the wall" things a doctor might say when he is in a hurry, either. I've been typing these doctors for years and getting good enough audit scores to have hung in there...who are they testing, me or the editors? And why? I have not asked to become a clinical documentation specialist. I'm in no way qualified for that, for heaven's sake! If Nuance has promised this level of proofing, we are all in big trouble! Who can type without second-guessing every phrase uttered? Why would Nuance want to slow us down this much when all they do is harp about line counts? They have definitely upped the game again, at least on me!

Unless there is a discrepancy in the patient's demographics, I send it through.

It's not worth my $10 per hour to find all these doctor's mistakes.

FYI - If a doctor makes an error on a dictation, it is HIS error, even per FIESA standards. There is a "critical dictator error" that QA uses in cases where the doctor made the error.

Use that information to your advantage.
I simply do not have that kind of faith in the newbie QCs, or even - the experienced ones, to do that.

hyphenated, Dr. Jones-Paperlapbabosalu and the dictator spells Jones, and of course you can't find it under Jones because it's under the part he/she didn't spell! Another personal favorite of mine is when I'm working on an unfamiliar account (which happens to us all the time) and the dictator states send a copy to Dr. Zippy, so I spend valuable time looking for Dr. Zippy, and there isn't one, only to find out it is the doctor's nickname, not even his real first name! Yep, I'll just put Dr. Zippy on the list for you doc, no problem! Idiots!

you wanted to quit mid shift?????  Sigh.  I had one of those yesterday.  Everytime I see his name come up in my cue I cringe!!!!  He is horrible.  Horrible accent.  I did notice though after doing about 3 or 4 of his reports, my ears becames "trained" on him and I get better with him as time went by.  But still, he is one that just makes my momentum (sp) drop drastically!  Just venting!!!  Thanks for letting me share. ...

Coumadin 30 mg per day. Now, I want to make sure I'm not dreaming here, but that is completely incorrect, right? Maybe I'm having a brain lapse (been an MT for 20 years) but I want to make sure my thinking is correct that this dosage would probably be lethal? I'm going to leave a blank for the dosage, but I just wanted to double check with my MT friends.  I have listened to it many times, and she is positively saying "30 mg per day."   TIA!! ...

I am just curious as to what speed setting to most of you keep it on when doing ASR?  I try to keep mine between 8 and 9 unless it's an extremely slow dictator.  I would like to increase my line count, but I'm afraid if I  speed it up too much I might miss something. ...

I know this is petty, but for some reason, it grates on my nerves to hear:  blah, blah Period space space.   Or Colon space space enter.  I mean, please, are we considered that incompentent?  but on the other end of the spectrum we have those that don't say any punctuation, which is absolutely fine, but a pause would be nice.  It's not necessary to dictate a whole report in one breath. ...

Just need to vent about my ESL doc who decided at 11 last night he needed to dictate all of his H&Ps for the week.  He's not very good at his best but his yawning is giving me tons of blanks and it drives me crazy not being able to get it, so have to listen 20 times to try to get one extra word, which of course takes much longer than his 5 minutes of dictation should require!  Really wish he would pick a weekday to talk since I only do weekends for this hospital! ...

Mumbly dictator First he clearly dictates a drop-fem test of which I am dumbfounded. As well as on an imaging report post total hip arthroplasty he dictated what sounds like he is stating.." Why aren't the dictators audited and scored for their poor dictation?  We can take points off for background noise, use of speaker phone, use of cell phone, static, chewing, yawning, side conversations, changing their mind, not pronouncing numbers correctly and so on.  Quite a few dictators would be on Performance Management CONSTANTLY and then would no longer be allowed to dictate until they straightened up and did it right.  One could only wish!    ...
