Have a doctor that doesn't like to dictate the . (point) in his cardiac sizes like 2.5 stent, he dictates 25. All his measurements are like that, I am a fan of you get what you give any thoughts?

If you know what the correct measurement is, then type it, because it will save you the grief of being reamed out by QA and made to sound like it's your fault he didn't dictate it correctly. I have a couple of docs like that too and I just use common sense, because when I let it go as is, I stress over it and that is worse. IF you get called on it, then I would definitely do it the way he dictates, but for your own sanity, play their stupid game and correct it. If this company wasn't such a "killer" of their MTs then it wouldn't be an issue, but they are!

If I know for sure where the "point" goes, then I put it in. If I'm not sure, I just type as dictated, spelled out. (Two-five stent.) Can't much argue with that, it's not incorrect--it's unclear, but not incorrect. It's definitely better than putting 0.25, 2/5, etc.

I like to think the reader would know, or at least spark them into realizing that they can't just do that and expect us to know everything.

Yeah you would think that some sort of common sense would prevail, but just read the post below where they are discussing ASR and it is titled "it is our fault." You cannot win with this place.

my QC told me I had to change melanotic to melenic because it was "just wrong." However, when I changed good to well to correct the English, that was changed back. Again, you cannot win.

I usually cannot complain too much about the ASR on my accounts. I am one of the very few lucky ones....I know. However, this is certainly not the case today! I was feeling really crappy about a report that took me forever to get through because the dictator was so terrible, not to mention I was already feeling down because I am already on 100% QA for failing a recent audit, so I knew they were really gonna tear me apart. Typed a friendly, clear, efficient doctor on an account I have never heard of before and will probably never have again, but he was dictating on a "pleasantly demented woman" and exam was "a little difficult" due to the fact that she "sang throughout the entire exam." Ok, it seems that this word has NEVER come up for me in the past. I have a new MD that states "we proceeded with the injection" in almost all his notes, and yet my spellcheck doesn't recognize "proceeded" or "proceed", "procede" or any form of that word. It keeps wanting to change it to "precede" or "preceded." What's even wierder is that I can't seem to find "proceeded" in ANY dictionary! Webster's has "proceed" but doesn't list a past tense for this word like it ...
