Has any one had a problem with QA MME's putting in text or teminology that was not in the report, eventhough you extesively went over it and proofread the report numerous times?

I have had this happen to me recently, So I have decided to take photo evidence of the reports before uploading in case a situation like this happens again and show them to compare.

Is anyone out there brave enough to step up and tell me if this is a common thing for higher up MModal employees to do to get one "terminated" from their job?

lie back & take their lumps, as usual. IT IS NOT A HIPAA VIOLATION UNLESS PATIENT INFORMATION DATA/DEMOGRAPHICS ARE EXPOSED.

Think about it: How many sample reports, how many voice files have you listened to as a part of testing ... do you think the MDs dictated them as a favor to the MTSO hiring process.

These reports were also once part of a patient record ... yet, what sets them apart is the fact that there is/was no identifying patient info. THAT'S WHAT SETS THE HIPAA APART.

If someone at your doctor's office was taking pictures of your chart and keeping them on their Smart Phone or camera? If that phone were lost, stolen or misplaced, then everything on it has possibly fallen into the wrong hands. Do what you want, but be careful how you handle patient records or you will find yourself in a lot more trouble than a bad QA score. You all seem very quick to worry about patient records going overseas. Judging by some of the things I've read here, I'd say we have plenty of problems with confidentiality among Americans who don't understand the implications of using patient information for personal use.

If the MME put anything in there that was not there, it will show up on audit and it will be her fault, not yours.

There was a document I had were the report was entirely accurate and when goin into QA I noticed on that same report things were changed to make it look like I made quality mistakes by the MME. The thing was try made it look like I was one making the mistake and then it goes against you.

By mention of photo evidence one does not have to include patient info one could easily take the originally edited document blank out patient name and locations ect and still be able to compare that one with the QA as proof.

but if the MME made changes, on the audit it will show that she made them, not you. You can't see that when you review your report, but they can.

I would address it with the QA MME but I would not take photo evidence. If there is identifiable patient info residing on the device you used to take the pictures, that probably is a HIPAA violation. Can you e-mail the documents with perhaps a "before" and "after" and address your concerns that way? I'm a QA person for a private clinic. Speaking from experience, I don't have time to single people out in the manner you describe. Anyone vindictive enough to do that intentionally to get someone fired has some serious issues. I'd look into it but be careful about making accusations like this. QA people make mistakes too.

Go ahead and knock yourself out. It will not make a bit of difference. Why do you think they do it in the first place. Again, this company is out to kill or brutally injure every MT it has. They have absolutely no respect, no consideration, no nothing when it comes to employees. Instead of wasting your energy on this, find another job and yes you can be terminated for this, don't kid yourself. They can get around the UE, stating you did not perform your job up to the required standards. Be afraid, very afraid and move on. This to me is a green light to get out; even if it is for your own sanity! What does it take for all the employees of this place to see what it is truly all about and yet continue to take it. Talk about banging your head against the wall.

The UE in most states, including mine, will grant the "burden of proof" after termination to the employee (for quality of work issues). The quality of work issue is so arbitrary/subjective ... just think about it, esp. with MT. Given the education and expertise required of us, add to that the fact that we're paid less than MW, well... the ball is in OUR ballpark.

If you are terminated for quality issues, go for UE ... THEY have to prove either INCOMPETENCE for the job, or MISCONDUCT.

you do have that right. When a MT applicant is asked to test via voice files/voice recognition ... do you think they hire voice actors ... or are those actual patient dictations, without identifying patient information, of course. Cover your hindies, MTs, don't be intimidated by QA/MTSO, etc.

Just be careful that if ANYONE saw them or MM knew you had copies of protected patient records you could get fired for that! I also do not think this is "abuse." It just sounds like fishy dealings to me. I do see your point though in keeping track of your jobs to prove that you did or did not do something that QA said you did/did not.

Why not just look at which MME is altering your reports? If it only happened once, maybe it was a mistake. If it's happening more often see who is doing it and email your team lead. By emailing your team lead you are documenting that this is happening and they could look at the reports to verify. You also aren't taking photos of medical records this way so no HIPAA violations.

If you think more than 1 MME is doing this, or that your team lead is in on it, then I sort of think this is a little out there and paranoid....

As long as there is no identifying info on my photo shoot ... it's MY PC ... my screen ... touch t***y if the MTSOs don't like it. We have to cover ourselves too, nobody else will do it for us. It's our word against QA ... and QA holds all the cards. Like it or lump it.

It may, and most probably is, a violation of your employee agreement. You will discover that when you try to use the photos and the MTSO has grounds to fire you.

It is also difficult to remove identifying information. It is not just the obvious names and medical record numbers, but any other information that could be used to ID the patient, like combinations of particular diagnoses, dates, and locations.

I had a similar issue at Nuance. The error was brought to my attention. I knew I had not made the error. It was a report I had saved a copy (text, no identifying info) to compare after the report went through editing (approved by boss). The report had a major error. I offered to forward the copy I had on my computer. They declined to see it. I was then told they couldn't recover the version I had submitted before editing which I cannot believe to be true from their latest platform.
All might have been accidental, but I was given no opportunity to prove I didn't make the error.

No one is going to do it for us.  There is wage theft, minimum wage, changing of timecards, disrespect for our knowledge, ridiculous working conditions, bad management, and no communication.  MTs have been left completely out of the conversation, and they are the MT in MTSO.  How bizarre is that? I think everyone should just walk away from their keyboards.  We won't know what the result of an action like that will be until we take it, and hold a positive intent on beha ...

I say people (?) because I'm really wondering if they're human and if they possess a heart. Due to overhiring and mismanagement once again, I am OOW and have been many times all week. We are commanded to adhere to a schedule, but flex when there is no work and then make up the time, which then interferes with another MT's schedule. Are we not supposed to have a life? You people at the top and in HR are reprehensible. I could say so much more, but I am better than that. ...

I was just curious about what employees thought of Nuance on Glassdoor.  You read several US employee opinions and they reflect the morale listed on MTStars.  Scroll further down and read the opinions of the transcriptionists from India.  What a difference.  They are highly satisfied with their jobs and most have been working with Nuance for 6 to 7 years.  The US transcriptionists are very sad!!  What a difference..... ...

Of course, there is the threat of corrective action. They say it is a performance issue??? Someone jumps on this board, fires it all up by saying to use F11 for every ASR job, and then calls their CCM to say "read the MTstars board" and then they go check who is doing it and fires them, all the while the pot-stirrer looks all cutsie in the CCMs eyes and more work opens up for the pot-stirrer?Maybe it is my wild imagination, but it seems like all that "stop typing for a day" and if that does not work, then "stop typing for 2 days," and "use F11 for all ASR jobs," all feels so not ...

All the research I have done on this subject is vague, but it appears that SE and IC are almost the same with the exception of Social Security and Medicare being withheld and the employer's portion being paid for an SE. ...
