I started with a company recently, (within the last 2 weeks),  that was all straight typing (yea!), but verbatim and I do mean verbatim.  Most of the reports had the headers built in, and I did them in the order dictated.  However, there were certain things in the operative reports, where they wanted things put in a certain place.  I got that part, after receiving emails from the QA team and put the headers, where I believed they wanted them to be.  The headers that were not used, I took out (again, this is what the majority of companies want); however, apparently not this company, because once the report goes to QA, there are things that we as MTs do not get in the report and they are added by QA.   None of the emails were snippy, I was told that I was doing a great job, and I took these suggesetions to heart, even saving the emails to "have a point of reference."   The work on this account was very sporadic and I sent an email to the MTSO, explaining that I would like to have a secondary account added if possible.   She replied back stating that they normally do not do this while a person is on QA review, that she was out of town, but would see what she could do when she got back.  On Wedneday, I had the second account added, but nothing came through on my first account (the reason being that "QA was swamped," and they figured with putting me on the second account, there would be less editing.  I worked on this account for a day and a half, getting an email from the QA manager regarding a doctor that she was able to find (I had left a blank because it was in a large metropolitan area and this was a VA facility account).  I did a report on Thursday, after spending 20 minutes trying to find a doctor (he may have been a resident), sending a email to the QA manager and asking for the website where I might be able to find a doctor's name.  She finally sent me the website, I could not find the doctor, and had to leave a blank. 

The verbatim part comes in with the fact that if the sentence does not make "grammatical sense," we could correct it, but, they wanted all the "ands etc.," put in.  I typed a report where the doctor had stated "creatinine is, BUN is," and NORMALLY I would not put "is" in.  I did in this case, then get corrected that I shouldn't have.   This company as well, does everything via email.  The company doesn't even have a phone number listed on their website.  I guess to me, "verbatim" is a double-edged sword.  You have doctors, who "stumble," and if you put the stumbling in, then you are made to look like an idiot.  I was even corrected on commas and semicolons, etc.  I have been an MT for 32 years, worn the QA hat, and never once dinged or been nit-picky over punctuation.  As a QA, I looked for "blatant errors," and felt that as long as patient care was not affected, then, so be it.   There were also cases apparently with this particular company that "I didn't hear the little stuff."  Again, it did not affect patient care in general.   I felt too, that this company could have provided the resources to be able to research doctor names, etc.  with this being a "speciality account."  I know how to reseach, I can phonetically spell and generally, can find what I am looking for in a short period of time. 

Bottom line was, the fact that on Friday evening (of course after close of business), I get a "termination" email.  I am finding that more and more of these companies think that you should know everything in less than a day and be "up to speed," in about 2 days.    All of this for 5-7 (and if you're lucky now, 8 cpl). 

If I weren't in my late 50s, having to learn Spanish - FLUENTLY to get an outside job, I would.  I am suffering from arthritis, was born with a slight disability, and have even had an evaluation by a neurologist because of my disability, and the fact that wtih most of these companies and VR, you have to use keyboard commands, which I am unable to do, along with the ridiculous line requirements per hour.  I tried to explain to this doctor that most companies want you to use a keyboard and not a mouse for VR, and I really believe, he did not grasp the concept of what is TRULY going on in the MT world.

I too was terminated after about two weeks of QA nit picking, and inconsistencies between the QA people...one saying "do it this way," and the other one saying..."nope, that's wrong...do it this way." In my "termination email" it was pointed out that I had made a blatant error of having used "plane films" instead of "plain films", that I had done it correctly in my "test" with them, so why did I get it incorrect in regular dictation. Well, I had saved my "test" from them and went to find the part where they said I had done it. I was shocked to find that the Diagnostic Studies portion of the test had only stated. "Xrays dictated in a separate note." There had been no mention of plain films, plane films, or whatever in the entire dictation!! How could I have a blatant error of doing that when it was not even dictated...test or not?! There were other things too, but I did not fight the "termination." I figured if they were going to be so inconsistent on things, I was better off without them anyway, but your company sounds exactly like the one I was with! I feel for you!

I have over 30 years of experience and got the same treatment. After two weeks, I got a call 30 minutes before my shift was to begin telling me I had been terminated. What happened to 90 days! Certainly, in 90 days, I would have been faster and better on the account, but two weeks!!

I was dinged by QA once for typing Walmart instead of Wal-Mart. I was also dinged for typing sunblock as 1 word, since QA insisted that it's 2, and I don't feel that we should have to waste our time looking up whether it's Walgreens or Walgreen's.

I remember once when I spelled trash can as one word in a report. It was the only error that the anal-retentive QA person could manage to find in the report, but because the report was very short, it resulted in a QA rating of 77% for that one report.

I understand that it is important to be right, but some words in the medical report are just more important than others. However, as a former nurse, I never came across a situation when Walmart versus Wal-Mart or trashcan versus trash can ever made a difference to me or to the care of my patients. As far as that goes, whether or not a generic drug was spelling with initial cap or all lowercase didn't make a bit of difference either. Most doctors that I knew couldn't have cared less about this stuff, either.

So, in such instances as this, I think we are just dealing with a big bag of baloney, bologna, salami..., along with a few QA individuals who apparently have nothing more important to worry about. Actually, I think their situation is quite pitiful and pathetic, yet they seem to be so very robotic and powerless. It's so sad.

When I worked inhouse, directly for a group of 9 doctors, I was never, ever bothered by them about such nonsense. In fact, the other MT and I ALWAYS used an uppercase on all drug names, brand name or generic. There were even a few times where we spelled some medical terms or drugs incorrectly (we had no spell check), and all they did was cross it out and spell it correctly.

BTW spell check says that trashcan is 1 word. LOL

My company emails all the time about how important our job is, that our accuracy is vital to patient care. If I were getting paid more, I'd be happy to look up all the stupid spellings they wanted, like did the patient pass out in a Walmart parking lot or a Wal-Mart parking lot. Sheesh. Talk about swallowing camels and choking on gnats. If our job IS so important, and managers talk like we're about one step down from keepers of the holy grail, then we should get paid like it. I bet my garbage collector makes more than I do, and my accuracy could make or break the quality of a patient's care?? I'm grateful to have a job, but just making ends meet is not much incentive to dot every I and cross every T. More and more I find myself wasting time looking up addressess and doctors' names. That's money down the drain when you get paid by the line.

Since you have seen a doctor and you have arthritis and a disability from birth I wonder if you would qualify for Social Security Benefits. It would worth a try. Just have everything documented. My sister worked as a lab tech drawing blood and then her son got killed and mentally it took such a toll on her she could not focus anymore on her profession. She has struggled and it has been 3 years now and a little better but not much. She went on line and filled out papers, then they contacted her and wanted her to see a doctor that works for them and she did and she qualified. My sister is in her late 50s. She is also a slight diabetic and has sleep apnea too. I am not sure if this made an impact, but I am thinking it did. It is worth a try. I have had 2 major back surgeries and foot surgeries. Now I am having trouble with a pinched nerve in my neck. That pain is also excruciating. It is all I can do to sit here. I used to work 40 hours a week and I was hurting so back I have had to cut it down to 30 hours now. I have thought about applying. I have worked since I was 17 and now am 62. People think it is an easy job to sit here but as MTs that is not true, and I also have severe right foraminal stenosis which is getting worse, if it is not my back, it is my neck and if that not it is my wrists and forearm aching. MT is getting worse and worse with such low pay even for a seasoned MT with years and tons of experience. You might want to think about looking into that. Good luck.

I actually had a disability evaluation done by a neurosurgeon done about a month or so ago. I went straight throuugh an attorney. However, the doctor determined that "I wasn't disabled enough." I was born with 1-2% cerebral palsy on my right side and because of the stiffness and spasticity, I have a hard time with using the keyboard when it comes to VR and sitting now for the long periods of time we have to sit has increased the pain in my lower back to the point, where I have a hard time even walking a half a block.

I tried to explain this to the doctor, but no such luck. He didn't quite grasp the concept of what all was entailed but that is JMHO. I haven't given up yet and hope to find an attorney who is more local to my area to take my case.

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested."  I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..."  So I decided to omit crappy and just say "she is still sounding pretty congested."  QA w ...

OK....this is what really pisses me off.....we are asked to "know" 75 pages of acct. specificis, 20+ pages of MTS specifics and the BOS, PLUS each individual acct. has their own way they want things, plus being "English Majors" for these reports and you are only paying us 4 cpl!!!?   I used to LOVE this job field but am really starting to hate it.  Been doing this for 15 years.  I hope it all gets outsourced to India so the docs really have something to complain about, then I ...

Gotta vent while awaiting manager response.  I do this report yesterday that required a cosigner, who I could not understand the name of.  The resident goes on to say the name of the cosigner again at the end of the report, but I still could not understand it (ESL).  I pend the report because of lack of knowing who the cosinger was and today I did one of those wonderful CPSE cirtical error emails because I did not put in the cosigner.  It gets better, the QA puts in ...

I just have to vent in the middle of this dictation where the dictator, a nurse, whispers the whole way through the dictation (not just this one but all of her dictations) and says "uh" at least 4 or 5 times per sentence in this very long dictation. I actually like the job. The dictators are fine. I'm a month into the job and now in the comfort zone, past the breaking-in period. But this is my first time doing voice recognition, and the pay is too low.  I really didn't have any experience in knowing an acceptable rate for VR. I just have to vent.....the thing that I cannot stand most about this job - even more than the pay, the lack of work, etc....is when the dictator CHEWS GUM when he/she is dictating.  The sound of someone smacking their gum, whether in person or on the phone, makes me CRAZY.  I am positive they do not do this in front of patients or other doctors...but since we aren't 'real people' to them...they just smack away.  I can barely get through the report.  OK.  ...

Venting session! I am a part-time work-at-home transcriptionist employed at a local hospital as "resource".  When I started a couple months ago we had over 1500 jobs in the "cue" with mandatory overtime ... then along came EPIC.  The regular employees are working in 2 hour shifts so everyone can get a few reports in ... having under 75 dictations has become the norm and there isn't enough work for them ... and I have had no work for 6 weeks.  ...

I just came across "Benzapril" listed as a medication that Speech Wreckignition picked up.  Granted, that is how this doctor pronounced it, but it should be "benazepril."  (Also notice that the original was capped while the correct is lower case.)  Now, I've seen this a couple times, and from what I understand, the speech system can't just "make stuff up."  Someone has had to have physically type that out in straight transcription (either when it was learning) or c ...

What Does Verbatim Mean In VR Reports?

This is almost/sort of a poll. I need to know what verbatim means to you in VR. Do you change any wording? My understanding of verbatim is word-for-word (Merriam-Webster dictionary) unless a sentence does not have proper structure; if the doctor rambles on and puts the word 'and' between words where there should be a period, etc.  (Not included in this is changing mg to mcg in a wrong dosage, changing the medication itself if wrong, or other major medical erros&nbs ...
