On one of my platforms we are required to copy and paste things like the social history, ROS, medications, etc., from prior reports. While OF COURSE it is our responsibility to be sure it is correct, often in the rush to keep line counts up, people are cutting and pasting errors. We were made aware of this with a particular dictator a while back, that everyone was copying and pasting an incorrect word that the speech rec put in that made absolutely no sense in context. But this is what you get when fast quantity has replaced thoughtful quality.
