Medical Diagnosis for a Dental Procedure - Donna R

I could really use some help please. I do insurance billing & coding for a Dental Office however we dont do any medical insurance filing. I have a patient that had an accident and has filed his own medical insurance. The insurance reo called me and wants a diagnosis code so they can process the claim for him.  He has a fall that fractured a few teeth. We extracted the fractured ones and did a fixed bridge.

Can anyone guide in on what the code would be?

 

Updated:  I'm sorry I may have misunderstood the Rep, however I am under the understanding that she only needs 1 code for a diagnosis. I have already sent her the ADA form with the dental codes.
