If after that, say in mid-February, then I would look at your deposits and add the amount and/or your best estimate, and then tell your CPA that you received no 1099 and they will take it from there. You can tell IRS the company name after April and they will investigate it. The company can be fined.

I experienced this last year, but it was not for an MTSO.
