This may vary from one supervisor or account to the next but I have never been picked on for "the" or commas. Commas will be an issue if you add or leave one out that changes the meaning of the sentence. And some dictators get snarky if they always say "the patient" or just "patient" and we add or subtract the "the," lol. This is especially true in verbatim accounts. But in my decade here, it has never been mentioned to me in an audit. That said, we get audited frequently, some of us several times a week, and they expect you to score 99.65% or higher. So yeah, even if it isn't every last comma or "the," it is a lot of detailed work for very little pay.
