EMRs with point and click and canned templates for PEs, ROSs, etc. and not about ASR. I think ASR is here to stay, but the quality of EMRs does need to change, the ones where the provider or an assistant is checking things off and self-typing into it. I work for an office doc, and I see some of the sloppy work being put into them by the office people and the docs themselves. That's what the OP is talking about.
