Dictated in a list of diagnoses: Ventricular tachycardia.
ASR typed (swear this is true!): Acute kidney disease.
Makes no sense. Totally different sounds. I guess what I'm saying is don't take your eyes off the screen even long enough to reach for a cup of coffee.
