That is hard to answer. It depends on the accounts you work on. There is the ratio of 1 minute of dictation equaling 10 lines. If you have slow talkers or back trackers (scratch that), your ratio will be less than 1 minute = 10 lines (might be 7 to 8). You are better paid the by minute. If your ratio is 1 minute = 10 lines or higher, better pay by the line. I am not familiar with per page pay in transcription. Best pay would be per hour plus incentive depending on production. That way, you would get paid regardless of poor dictation practices, which are rampant nowadays.
