First of all call the Dept. of Labor. They cannot force you into being an IC without your consent. That is against the law. It is also illegal for them not to pay minimum wage. I would not go IC status or sign anything, talk to the Dept. of Labor first.
I will be going IC but with another company and I have been an IC before years ago. But, I am staying with Imedx until I am let go because I want the unemployment and that is something they do not want to pay either. According to the DOL they are doing illegal things and if you sign something you are agreeing to what they want, even if you do not want it.
