I don't punish harshly for this. I mark 1/4 point the first time the mistake is made in an MT's QA, and if it's made subsequent times I merely make a note of it, but take no further points off. (I tend not to ding an MT multiple times for the same error in a batch of reports I'm QA-ing.)
