If there are doctors at multiple hospitals with the first and last name the same but with different spellings, then I would something for the hospital that is not my main one. So if it was Jon Smith at hospital 1, it would just be josm, but if there is John Smith at hospital 2 (say St. Joe's) it would stjjosm.
