While dictating at the noisy nurse's station/emergency room with machines constantly beeping, nurses partying yelling and/or giggling, overhead speaker announcing something, and your phone/beeper going off... putting your hand over the microphone WHILE STILL DICTATING does not help neither does putting your mouth directly on the microphone mumbling into it to supposedly drown out the background noises.
