There would seem to be no legitimate testing or applicant evaluation reason to ask you to redo the work in the template. It is very possible, even likely that the test was being used to get free labor.
This is not unheard of. Others have posted in the past of similar experiences. Some of them have been asked to transcribe several quite long documents that were from the current date, and some have even included patient demographics.
At least they only got one document from you.
I hope you are able to find the right fit with a good, trustworthy company.
