The owner?? Surely, this person knows the rules of HIPAA. Sounds like a whistle-blower situation. If you tell her/him what you know, there's a lot that could happen. Assuming the violation is serious enough to concern you (enough to work it out here), my guess is you have to tell somebody. I'd start with the owner, let them know what you know, that you want to help resolve it, and that you're not throwing them under the bus by going straight to the hospital/clinic. If you simply go over her/his head and straight to the facility, you're going to be seen as the bad guy. To keep a working relationship still a "working" job, I'd start with talking to the owner, see what you can do in a helpful sense.
