That is an overly simplistic view of coding. Coders do not code from the listed diagnoses, nor are they impeded by spelling and punctuation mistakes. We don't code only from typed reports, either, but from the entire record, including all the handwritten and doctor-typed material -- symbols, abbreviations, acronyms, misspellings, incomplete sentences, no punctuation, ERRORS, and everything else.
In fact, one of our functions is to identify errors in diagnoses, omitted documentation and diagnoses, and other problems, and to get the doctor to correct it. We do this by reading the entire record, including the labs, and determining what SHOULD have been documented. Considering that, it is ridiculous to say that an MTs spelling errors prevent coding and reimbursement.
It is also very ignorant to say that "fraud happens" when there are coding errors. Fraud is intentional. There is no intent in an error. Insurance companies pay for errors all the time.
