They do not save money by hiring directly because if you are a hospital employee they have to pay health insurance, PTO and provide other benefits, that's very expensive. The only way they might do it would be if you were an IC. They are currently getting the work done offshore for next to nothing. The quality is not bad enough to make them considering bringing it back on site.
