I feel the only wise choice (job-wise that is) is follow orders. I haven't been 'smacked' yet for doing so, although there is always time. I don't pick a billing number unless it is specifically stated or already in the ADT. I don't put in a date of service unless dictator said "procedure done on" or "date of visit is" very specifically. If dictator states the age wrong, I blank it. If there is only one Dr. Zirconium in the database and dictator spells it but doesn't give a first name, I type it in as new. CMA all the way at this point.
If I'm feeling particularly refrained from using my brain, I have occasionally made my discrepancy comment something like, "Sorry, I could be fired for assuming the patient's age (calculated from the date of birth you dictated) is correct since you went on later to say her age is XX. All I can do is leave a blank."
I don't know if a dictator ever sees that comment, or cares, but at least I have explained my situation
