But it's been awhile (2-1/2 years). I'll be finding out soon because I need to do it again. Not sure how decent MM is about it these days. I did return to my supervisor, account, and hours. I filled out paperwork which is very important and your doctor has a section to complete. It doesn't have to be your specialist or surgeon either. PCP works just fine. Best of luck to you.
