Some omissions - I would add to this...

Learn the difference between the patient's MR# and acct#, and enter the proper identifier into the system so I don't have to look up the demographics on every report you dictate.

Pay attention to the format conventions used by the facility you're dictating for - you can see them in just about every report dictated by other people who follow the rules.

No speaker phone - EVER!

Give first AND last names of physicians you mention, want copies to, etc - at least the first time they're mentioned. Even when the last name seems unique to you, in large facilities there might be 2, 3 or more Dr. Shanawanadingdang's.

If you don't know how to pronounce a drug, an instrument, a procedure, spell it.
