You're doing a report on a male patient:
"He is status post abdominal hysterectomy and bilateral salpingo-oophorectomy....".
Bwahaha, you laugh heartily, calling the doctor several inventive names that you've been saving up...and you don't wait to slap a dictation dscrepancy note on it. You do it now, so you won't forget.
Then, much later: "He is transgender, female to male."
