This wasn't mine, but read this audit on our daily list this morning:
MT put: Ibuprofen 180 mg every 6 hours needed for pain.
Auditor put: Ibuprofen 180 mg every 6-8 hours as needed for pain. Then gave this comment: Poor quality voice file made this text hard to hear. Audit Score: Feedback 0.0.
My gripe is that this is so subjective!!! I'm glad the MT didn't get any points off, but chances are, if I had done the same thing, it would've been a 3-point medication error. The scoring system is total BS!!!
