This has been bothering me. I have one provider who says in her ROS, for example: "The patient denies nausea, vomiting, diarrhea, or melena."
I think the "or" should be an "and," as the patient is denying each AND every one of these things, not this one OR that one. Of course, I understand what the provider is implying, but I think the correct word used should be "and," not "or." What do you think?
If anyone can give and answer and back up with a grammatical rule, I would greatly appreciate it. Thank you, Linda
