Hello, am having trouble with this sentence. I don't think it's "iron arm", I think it is somthing to do with his iron level though, as further in the report referring back to the admission, iron is mentioned again as elevated above 90. Any ideas? Thanks

Hi. That really is the full sentence. Here is the containing paragraph:

CLINICAL COURSE
This is a 21-year-old gentleman with end-stage renal disease who is dialysis dependent, who came in with a history of bleeding from the right upper extremity for 1 day. His iron??? the day before and noted it to be elevated, and his physician had tried to contact him. He had stopped taking his Coumadin as per his physician̢۪s telephone message, but he developed bleeding overnight for which reason he came to the hospital.

I also think it is INR. The patient is taking Coumadin and INR levels are one of the things they routinely monitor to adjust the dose.

The sentence is "Normal iron studies with a (s/l version) of 25." Sounds a little like version, burden, something along those lines.  Doctor is almost shouting so the audio is distorting a bit.  I know I'm going to kick myself when I figure this out.  Any thoughts?  Thanks! ...

I have alway been told when in doubt, leave a blank. Resident did not give attending physician's name. There was a doctor's name in the hearder as attending, but I  was not sure so I left the attending line blank at the bottom of the report. My boss calls me and tell s me to go on a limb and guess in the future.  I do not like those ethics because it leaves me up for problems in the future.  Comments welcomed. ...

The doctor I transcribe for sometimes says "Associated Physician" and sometimes says "Associative Physician" and then lists the doctor's name.   Sometimes he will also list a "Primary Physician."  Just curious to see if there is a difference between associated vs associative and if one would be better than the other in this situation.  Any thoughts? I admit, this is a huge pet peeve of mine.  The consulting physician is the one who goes to see the patient at the request of the REFERRING physician.  I wish I could get some docs to read this and state the difference.  If I was typing, I could just correct it right away but I have to correct it on the VR.  ...
