I remember our CCM telling us a while back to do something like put the number sign at the beginning of each item and it would automatically convert to numbering when it uploads? Does anyone know exactly what we're to put? A number sign, period, and 2 spaces? ...

Why the constant slang and abbreviations in diagnoses and impressions? Especially when it could mean more than one thing ( for example I and D) and it cannot be gleaned from the record.
