what is the best way to start, let's say for example in the EXTREMITIES:  doc says, 2+ peripheral pulses.  I know we should not start sentence using numerals, so what is the best way to start this sentence.  There is 2+ peripheral pulses? I would like to see a variety of ways this could be started.  Thanks.

How about: EXTREMITIES: Peripheral pulses 2+ - just changing the order is all.

While doing a physical exam of the back, the doc says: "On the left posterior midclavicular line, approximately s/l two-twelve region or 2/12 region, there is a 1-cm mass."  How is the "two-twelve" part expressed?
