Heart exam, doctor dictates: "Heart: Regular rhythm without murmurs or gallops. 2 clicks heard at the apex"  I don't understand the connection between "2" and "clicks" or what punctuation to use??

I'm new to this so any help would be much appreciated!! Thanks!!

Do you hear dictated "clicks" or "click". Could it be that something is being said about the second heart sound (S2), and then he address the a "click" (abnormal heart sound) heard at the apex?

He definitely says clickS. And yes, I thought he may be talking about the 2nd heart sound, but the 1st part about "regular rhythm with murmurs or gallops" is very clear, then he says "2 clicks heard at the apex". Wondered if he means the 2nd heart sound was a click and if I need to transcribe something like 2: Clicks heard at the apex. I don't know!!!!

In that case, I think your first instinct was correct. The second heart sound is the "dub" in "lub-dub"; it wouldn't, as far as I know (many years as cardiology tech but I learn new stuff every day) be an S2 click.

Ergo, would do as you have, with the only modification being to write out the "two".

OK, I think I'll go with that!! The "2" is very distinct and sounds like he is saying it almost like in a numbered list...

Gosh, I really don't think so. He says "2" like it's next on the list, but he never said 1. And it's definitely a strong 2, pause, clicks heard at the apex.

AHHHHH!!!! So frustrated!! :)Thanks for your input. I'll go back and listen again just to be sure.

2/6 heard at the apex? That doesn't make sense though, does it?? There's definitely not enough syllables for systolic murmur to be in there too...

He already says there are no murmurs - Two clicks heard

While doing a physical exam of the back, the doc says: "On the left posterior midclavicular line, approximately s/l two-twelve region or 2/12 region, there is a 1-cm mass."  How is the "two-twelve" part expressed?
