History and Physical Examination report - JRG

When arrival to Baptist Hospital, he was in significant respiratory distress, ABG’s were checked and initial pH-7.25, pCO2-69, S/L "it appeared" to 128, bicarb 30 and S/L "IO2" or could it be "SO2" to 40%.  Thank you

It is far more likely that the ear tranformed "on" to "when".

I'm trying to help her answer her question; not nit-pick the other text (proofreading will be done I am sure, but I'm not on the QA clock right now, so I am going to focus on the QUESTION only)....none of us have an idea of what was dictated. Chill a bit. It's ok, I promise. :)

the tests included in ABGs are:
pH
PaO2
HCO3
PaCO2
and BE (base excess)

Maybe if you listen again you will hear them in context. Hope this helps. :)

While doing a physical exam of the back, the doc says: "On the left posterior midclavicular line, approximately s/l two-twelve region or 2/12 region, there is a 1-cm mass."  How is the "two-twelve" part expressed? The lower extremities show marked peripheral varicosities.  There are also some excoriated lesions of the lower extremities with some mild scarring secondary to __s/l "thorabnitis".
