mild to moderate htn or plaque or anything - hyphens or not?

Lately, I have been getting dinged for not putting hyphens in mild/moderate to moderate/severe plaque or moderate to severe htn or mild to moderate anything.  I don't understand why we would need hyphens.  In that case, shouldn't "anything to anything" need hyphens?

If the "mild to moderate" comes before the noun, hyphens are used - Helper Monkey

Example:

The patient has mild-to-moderate pain.
or
The patient states his pain is mild to moderate.

The patient has left-greater-than-right osteoarthritis of the knees.
or
The patient has osteoarthritis of the knees, left greater than right.

The funny thing is, in the past, I've been told by QA to drop the hyphens... at more than 1 company. If you're getting dinged for this, my guess is that you might work for a company whose goal it is to drop your QA score to avoid paying incentive. Maybe it's just an overzealous QA person. Either way, it does not change the record and should not be considered a critical error.

Unfortunately, I do not have a BOS (and refuse to buy a new one) handy to give you their definite answer on this. Maybe someone else can help you out with this.

I transcribe radiology, and the following phrase is used frequently: "..... mild to moderate grade...." by one particular doctor.
