Where to place an addendum in a letter - forgetful

It's been so long since I've come across this that I don't remember how to do it. I just typed a letter with a cc, and now she added on an addendum. Where in the letter does it go? After the cc? Before the signature? Between the signature and cc?
Also, she said the date of the addendum. Where does that go? I recall writing "ADDENDUM" in all caps before the addendum, but how would I write "Addendum of August 1, 2012"?

[ In Reply To ..]Addendums are an afterthought, like a P.S., but should go before the signature line. In this way, the doctor gets to sign off on the original dictation and the addendum with one signature. A carbon copy always goes after the signature, and if a cc gets the original, certainly the same cc would be included with the addendum.

I'm not sure what the doctor is saying b/c he's speaking too fast. Before the following sentence he says Addendum (possible subheading), Addition or Additionally. "The end of operation is satisfactory." He talks about closing the wound before this and talks about estimated blood loss after this. Any ideas?? Thanks! ...

A few weeks ago, for no reason that I can find, I cannot get the first letter of a sentence to be in all caps automatically, (unless I do it manually.) The same for the letter " I " by itself when typed. It was always automatic and now all of a sudden it is doing this and I cannot figure out how to change it back. Has anybody else had this problem, if so, how did you fix it? Thanks, appreciate the help ...

I was thinking of using this as a replacement for typing/transcribing. For example when I hear the dictation, I then repeat it to the Dragon voice recognition software, and it types for me. (You need to read the post before this one for it to all make sense -- sorry!) Thanks, Abby ...

I work for them now and they are the most wonderful people. It is straight typing, no voice recognition AT ALL!! You can make up to 9-1/2 cents a line and they are not picky about how long you work.
