Report Capitalization Question - TXMT

When a doc is referencing a report in his dictation, i.e., please see the attached report, John Doe Lab Flow Cytometry report, John Doe Lab flow cytometry report or John Doe Lab Flow Cytometry Report, for further information. Which is correct?

[ In Reply To ..]Agree with NM-Old Woman, unless the name of the lab is "John Doe Lab", in which case those 3 words are capitalized, with the rest being lower case.

I have been going back and forth on this, but doc says, "He was also found to be hypertensive. Patient's hypertension was managed by *anesthesia* and a 12-lead EKG was obtained." My gut is wanting to capitalize anesthesia to show that it was the anesthesiologists in the Anesthesia Department who managed the hypertension rather than anesthesia itself, but then again, maybe not? Would it be safer to not capitalize it? ...

I have always been under the impression that when a department is used as an entity, such as "The patient was seen by the Pulmonary Department," that it would be capitalized. If it were "The patient was seen in the pulmonary department," it would not be capitalized. How do you know when to capitalize departments. I've always typed emergency room, radiology, cardioliogy, dialysis. Now its Radiology, Cardiology, Dialysis, emergency room.
