Psych Consultation - Stumped

At the end of the report the doctor says Axis I -Adjustment disorder with anxiety, moderate in degree. Then he says "New paragraph, Psychogenic amnesia", then he says Axis II . Do I skip a line after Axis I, then write Psychogenic amnesia? How do I set that up? Please help, Thanks.

I usually ignore nonsense formatting, partly because it can mess up the back-end formatting. If there is no rule anywhere about that, I would just type it as if they never dictated the new paragraph.

...I'm not real sure how your client wants it, so this isn't a cardinal rule. It gets confusing with back-end formatting, so who knows, but I try not to split things up like that.

Axis I - hc

I have always typed the diagnoses in list form for Axis I and Axis II unless the doc dictates a comma after each diagnosis. Example below will not let me tab over to line up the sentences that continue onto the next line, so I shortened them,but when the sentence does go over I tab it over so everything lines up. My guess is he just wanted the second diagnosis on Axis I lined up right under the first diagnosis and then says Axis II, meaning he is now listing Axis II after that.

Axis I: Adjustment disorder. Psychogenic amnesia.
Axis II: Deferred.
Axis III: Diabetes, hypertension.
Axis IV: Financial problems. Axis V: 55.



Word allows it to line up, but this does not. - sorry about that

working on a psychiatry consulation report and there are only a few headings dictated. "History of Present Illness", "Review of Past Medical Records", "Developmental Background", and "Diagnosis". Under "Review of Past Medical Records" there are several paragraphs dictated without headings and each one starts out with "Doctor (doctors name is cut off) diagnosed (or indicates) etc....., examples: Doctor diagnosed head injury, loss of consciousness, bilateral clavicle fractures, right scapula ...

Is there anyone here who can tell me why many of the companies who have psych reports only want MTs with 5 or more years experience? I've done a few psych reports and had no problems with them. I find them fascinating and would love to secure a job doing mainly those type of reports (if there are such jobs?). Just wondering why the 5 year requirement?
