Ortho question - "OR" treadmill fracture??? - Vikefantam

Posted: Dec 17, 2009

I've had an orthopedist mumbling his way thru his dictation all morning.

I now have an patient status post a trimalleolar Fx.  There is concern of a stress Fx.  She is 50 years old if this helps any.   She has tenderness along 2nd metatarsal shaft with mild swelling in area.  There is also mild tenderness in medial malleolar region.  She has good motion except for dorsiflexion.  X-rays show ankle is in good alignment with no stress Fx.

My problem is her impression since we are allowed no abbreviations.  He is saying she is status post what s/l and "OR" (not ORIF) left treadmill fracture and ankle fracture from last August.  She also had a stress reaction of the 2nd metatarsal.

He is going to give her an injection in the anterolateral aspect of her ankle.  I'm ready to scream.  And I wonder why I fracture so many teeth.  It's from grinding them from doing so many doctors like this in a row.  (eye roll).

Any help would be greatly appreciated.

Thank you!  Hugs!

Could treadmill fracture = trimalleolar fracture? - truffle

You said he was a mumbler.

Maybe status post open reduction (OR) left trimalleolar fracture...

fracture - repeat

I believe the other poster is correct. In the impression section they will most likely repeat some of the history of information that was already dictated, so as noted in your first line the paient had a hx of trimalleolar fx, so that would probably be it, as he is restating information and recapping information on the patient. Always check with the beginning of the report to see if it would sound like somewhat already dictated.

Treadmill Desk
Nov 22, 2013

Hello!  I have been an MT for going on 10 years now and have been thinking about a standing desk.  However, I have a tendency to stand in a way that I end up hurting the outsides of my feet.  I had an idea for a treadmill desk, but I'm stuck on the foot pedal issue and noise level.  I did a search and found a post on MTstars by someone who said it wouldn't' work because treadmills are too loud and one wouldn't be able to hear the dictation.  My work a ...

Segond fracture:  This was a new one for me today and thought I'd share.  See Wikipedia link below for more, but basically it's named after a French doctor (Paul Segond) from 1879.  It's a "type of avulsion fracture (soft tissue structures tearing off bits of their bony attachment) of the lateral tibial condyle of the knee, immediately beyond the surface which articulates with the femur." --Wikipedia.  My case involved a skiing injury where the binding did ...

Good evening everyone.  So, this patient had a galeazzi fracture of the forearm.  The doctor says about his film study:  "On the lateral film I think there is a little angius in the distal fragment, a lot of segment comminution."  Now on the tape it sounded like segmentminution, so i'm wondering if angius is two words also?  This may help also:  "comminuted fracture through the diaphyseal portion of the distal radius", and "Galeazzi fracture with some disruptio ...
