This large facility was loaded with terrible dictators, highly specialized terminology, and individual physicians with their own preferred specifications, let alone the account had detailed and constantly changing specs. ...

Dictated: Bone marrow aspiration and biopsy showed an increased number of small lymphoid cells replacing the marrow in a continuous interstitial and sheet-like pattern. Iron stores were present. There was decreased erythropoietic and granulopoietic activity. There were occasional s/l "neck-yu-TAR-ia" sites. The lymphocytes seen had mature chromatin.
