do you spell out PICC line in discharge diagnoses? - i do not want to overdo it

Do you spell out every little thing, including PICC line and IV, or is that just such common knowledge? I'm already expanding MRSA, but I wonder about common abbrev.

I had gone ahead and left it as abbreviations and uploaded the job. I figured anyone reading it should know what it means. Thanks for the general rule about expanding the actual diagnosis.
I am transcribing a report for a PICC line placement and at the end the doctor says that he places what s/l anestasol Steri-Strips. a-nest-a-sal.
Anestasol Steri-Strips were applied.
He could mean anestasol and Steri-Strips but he doesn't say that (which of course does not mean the "and" is not needed!!).
Anyone know what this should be? Thanks! ...

ASR always used to type out PICC (not PIC) and disk (not disc). I just did a long report and throughout the report it had both disc and disk even in the same sentence. Also, we were told PICC is the correct abbreviation, not PIC -- so why all of a sudden can't ASR get these abbreviations/words correct? Why is it getting worse? How is it getting worse? ...
Doctor dictated "Mother is wiping pusy discharge from his eyes every 10 minutes since he awoke this morning." The word in orange, I would normally change to "pus-like", as I don't think this is how "pus-e" discharge is spelled. I keep thinking it should be "pussy", but that's clearly not how you pronounce it....LOL...HELP!!! ...

Hello, I was typing the "Final Diagnoses" part a discharge summary the other day. The doctor began listing them by number and for the last number of 9, he said, "Jehovah's Witness." How is that a diagnosis?? Shouldn't those words be in the body of the report instead?? What is the correct way/formatting of handling that when it comes to religion??

I'm having a problem with this part of the diagnosis as she doesn't specify to go on to the next number, as she does with the rest of them, but I've never had someone that adds the procedures used to fix the diagnoses in this section. This is the sentence as she says it, "1. Coronary artery disease, status post 3-vessel CABG in ___ vertebrobasilar insufficiency." The she says "number next" and goes on to the next one.
