He says-   No s/l "vavinski" sign.  Does he mean Babinski or Brudzinski??

Babinski sign 

1. extension of the great toe and abduction of the other toes instead of the normal flexion reflex to plantar stimulation, considered indicative of pyramidal tract involvement (âpositiveâ Babinski); Syn: Babinski phenomenon, toe phenomenon, Babinski reflex, great-toe reflex, paradoxical extensor reflex.
2. in hemiplegia, weakness of the platysma muscle on the affected side, as is evident in such actions as blowing or opening the mouth;
3. when the patient is lying supine, with arms crossed on the front of the chest, and attempts to assume the sitting posture, the thigh on the side of an organic paralysis is flexed and the heel raised, whereas the limb on the sound side remains flat;
4. in hemiplegia, the forearm on the affected side turns to a pronated position when placed in a position of supination.

Brudzinski sign

1. in meningitis, on passive flexion of the leg on one side, a similar movement occurs in the opposite leg. Syn: contralateral reflex, contralateral sign.
2. in meningitis, involuntary flexion of the knees and hips following flexion of the neck while supine. Syn: neck sign.

If doc is checking for meningitis, it would be the Brudzinski sign. Babinski is a test done on the sole of the foot.

Previous sentence is

"Cranial nerves 2-12, sensorimotor, reflex examination are intact."

My thinking is he said reflex exam is intact and in reference books it's called Babinski REFLEX and Brudzinski SIGN... so maybe he means Brudzinski sign.. ahhhh!

I would think if they were testing the Brudzinski they would mention meningitis etc.

I also just found a previous report with Brudzinski sign listed under the Neck exam heading- so likely it's Babinski I'm hoping :)

Babinski 


Extension upward of the toes when the sole of the foot is stroked firmly on the outer side from the heel to the front; normal in infants under the age of two years but a sign of brain or spinal cord injury in older persons


In medicine and neurology, the Babinski response to the plantar reflex is a reflex that can identify disease of the spinal cord and brain and also exists as a primitive reflex in infants.  When non-pathological, it is called the plantar reflex and a downward response is elicited, while the term Babinski's sign (or Koch's sign) refers to an upward response that is pathological in origin. When describing the patient's reflex response it is simply indicated as: Babinski's sign present or Babinski's sign absent.


 


Brudzinski


The Brudzinski’s neck sign or Brudzinski's symptom is a clinical sign in which forced flexion of the neck elicits a reflex flexion of the hips. It is found in patients with meningitis, subarachnoid hemorrhage and possibly encephalitis.

When the doctor dictates about the gait and tandem gait, do you leave that under extremities if he dictates it there?  He usually says it at very end of physical exam, and I am unsure if he just forgot to dictate "NEUROLOGICAL" heading or what.  Sometimes he does dictate a NEUROLOGICAL heading and puts the gait there as well as other things, but then many times he dictates it at end of extremities and does not give separate heading.  What would be the appropriate thing to do in th ...
