Editing To A Deficit. A Consideration

As I sit here and edit/retype at 4cpl speech recognition for about 99% of my payperiods, I realize that every time I delete words, sentences, phrases, etc. for quality, for my MME's preference (do not be redundant, delete words dictated like significant for etc)... EVERY TIME I delete words, I am taking pay away from myself. These add up.  So, if I were to sit and delete 99% of a document, I have spent my time reading through and listening with my technical knowledge to gibberish ...

Ever think about how this also applies to 20-minute dictations that are filled with "sorry correct that to..." and "UHs, UMS?" Also working "to a deficit" to all of our detriments.  Don't continue to accept it. In an earlier post it was mentioned that there will be more activities and ongoing activities during and past MT Week. ...

When the doctor dictates about the gait and tandem gait, do you leave that under extremities if he dictates it there?  He usually says it at very end of physical exam, and I am unsure if he just forgot to dictate "NEUROLOGICAL" heading or what.  Sometimes he does dictate a NEUROLOGICAL heading and puts the gait there as well as other things, but then many times he dictates it at end of extremities and does not give separate heading.  What would be the appropriate thing to do in th ...

I really have a problem under the Physical Examination trying to figure out what goes where when the headings aren't dictated.  For instance, in this dictation, she dictates lungs, describes her exam of them, then goes on to say, "Mass in the back and the left paraspinous area is measured..."  Would this stay under the lungs or would I make a new heading...if so, if "Back" an acceptable heading? Next, she talks about the pelvis...would this go under a heading with a "Pelvis" labe ...

This is an ESL doctor who is a very good dictator, but for some reason this part always trips me up.  She says what sounds like:  "Cardiovascular:  S1, S2, plus 0.  No murmurs, rubs, or gallops." It's the plus 0 part that gets me.  Any ideas?  ...
