I've been at this forever it seems...  :)  I'm doing an ESL, and I'm a little stumped.  The patient has elevated BUN and creatinine and stage 3 chronic kidney disease. Here's what he dictates with regard to her medications:

"She was on Benicar and Celebrex, and her BUN and creatinine have gotten worse recently.  I stopped both of these 2 medicines and switched her to _____ (s/l Adoquin or Adocrin), which does not have the sulfa in it, as the patient has got a sulfa allergy also."

My mind is just blown.  I'm not a newbie at this by any means (17 years under my belt), but this ESL has stumped me.  I'm sure it's probably something easy, but at this point I haven't got a clue as to what to even look for.

I found Edecrin, which COULD be what he is saying, but I also found that it is only a diuretic.  She does have hypertension, CAD, atrial fibrillation, and CKD. 

Any ideas?  Any of you other seasoned MTs out there have any suggestions for me?  I would greatly appreciate it any input!

He says this same medication 3 different times in the report, once in the sentence I originally posted, once in the medication list, and then again at the end in the impression and plan.

This is what he says in the impression/plan area:

"1. Congestive heart failure. I am going to do a stat chest x-ray with a BMP and CBC. Unfortunately, we do not have the _____ in the hospital, so I am going to start her on Bumex...."

The blank s/l Edecrin or Elequine BUT the medication could start with an A and he tends to lisp or miss the R sound in most of his words.

Atacand at first. He's really not too terrible to decipher once I get used to "the way" he pronounces words, especially the "R" and "L" sounds in any word.

I did FINALLY figure out he was saying Edecrin, which is a drug I have never transcribed in all my 17 years. I had to research it some more once I got to the end of the report. He said it again for the EIGHTH time in the "discussion" section of his report at the very end. When he said it there, I figured out he was talking about giving her a diuretic and discontinuing the Benicar completely.

I spent almost an hour on that little 4-minute report, but I just can't send my reports in with 8 blanks for ONE medication name. That is something that really bothers me for some reason. I know that's what QC people are there for, to help us with these kinds of things, but I also know from all my years of experience that if I have to look and look and look for something, I usually remember it WAY better the next time I get it than if someone just gives me the answer to my blank in a report AFTER I send it in.

I was just stuck this time and thought if another experienced "ear" could "hear" something I might be missing, it'd help me get through the report just a little bit quicker....especially since I'm making next to nothing editing these dang things! :)

I don't know why but your answers made me LOL! I guess I'm just so tired of my job lately that my sense of humor is going off the deep end too. :)

I actually found Eloquin when I searched the first few times. It kept popping up no matter how I spelled what it "sounded like" he was saying.

He actually said this medication EIGHT times by the time I got to the end of the report. I might have sent it in with 2 or 3 blanks for the same medication, but I refused to send it in with EIGHT blanks in it. I just have this thing about my work being accurate and it bugs the heck out of me when I can't figure something out for myself.

By the time I got to the VERY end of the report, he did a "discussion" paragraph, and in that paragraph he mentioned something to the effect of putting the patient on Bumex IV for diuresis because the hospital pharmacy did not have "Edecrin." I figured out it was Edecrin after again searching this medication and finding a website that stated it was used for diuresis and "sometimes" for high blood pressure.

I spent 45 minutes in this 4 minute report, but I was determined to figure it out for myself without having to send it to QC for review. I find that if I frustrate myself enough looking for something and can finally figure it out then I'll remember those less than common medications when they're dictated again later on. :) It usually costs me my productivity and sometimes my sanity, LOL. But, it has made me a better transcriptionist over all these years I've been doing this because I tend to remember the things that were a "pain in the rear" to find. :)

told to take timed tests in school. If you don't get it the first time after a little checking, move on, and maybe it will be repeated more clearly later. Also, when I go back to check my QA markers after I have finished a difficult dictator, I can often understand them better, even on short reports.

AND our delightful ex-MQ company actually docks our pay for sending too many markers to QA so I sure as heck never send them much.

Did anyone else get the message from Massanotti?  What do you think?  Also, I went to medquist.com and was struck by the fact that they are marketing front-end dictation systems with SpeechQ, thereby eliminating transcriptionists altogether!  I am not surprised, any way to get us out of the picture will make them more money.  It just seems odd that they would tell us on one hand that they value us and then on the other try to get rid of us altogether.... ...
