H&P Neurology Report, lots of help needed - Mary triplet mommy

I have been on this report for 2 days now. I have managed to figure most of it out but I am really lost on one paragraph. After she dictates all the information under Physical Examination she goes into another paragraph describing more of the patient's symptoms. Is there supposed to be heading before that paragraph? Also, below is a few sentences from that paragraph that I just cant make out. Any help would be great. Thanks again.

Tone is normal no clog whealing. Degenerative reflexes are equal. Blanches are down going bilaterally. Coordination is slow but intact on finger to nose bilaterally. She has no poinate or drift however her left arm bobs quite a lot on outstretched hands.

Check your Stedman's Neuro words under "reflexes" for something that s/l degenerative. I've never heard that term before, but I could be wrong. I don't have my reference materials available right now. : )

I would guess degenerative = deep tendon - Yoda nee Oracle nm

Made a mistake up there. It should be __________ reflexes are equal. Sorry about that. And the blank might be "deep tendon."

I will check out the "degenerative" reflexes again. Looks like everything else you posted was dead on. You have replied before to my post and are always so helpful. How did you get so good at this. ;0) Thanks again!!

I am on this same report now. Chapter 14. I am having the same problem you had. Someone told me it may be babinski's are down going bilaterally. But it does sound like blanches.
My biggest problem is in the HPI section ...I cannot figure out.... She has never had electromyogram (EMG), but did have work-up for lumbar stenosis, she has had an operation to release a tethered cord and also fusion ________. could you help me with this?

I spend enormous amounts of time doing clervical work for free at the beginning of a report..looking up doctor names that are not on the lists we have been given, figuring out which is the date of admit, consult, etc.  If everything was programmed in and we just had to download a report and transcribe, we would be making a lot more and if the speech recognition would stop putting words down that the doctors are not saying so that we do not have to clean it all up we would have more time to ...

S:   The patient was last seen for continued follow up due to her history of myoclonic seizures and BNS for intractable seizure activity on 05/18/10. I'm an MT student and I'm struggling with this dictation. it sounds to me like he says "where he underwent a craniectomy, which removed the left subdural bled." However, the MT grading my papers says that bled and bleed are "the incorrect form of the correct word". I thought those were the only 2 forms of the word??  ...

MENTAL STATUS: The patient is awake, alert and generally very anxious, with pressured and thick speech. She is unable to sit still and is very ______ (doc spells it out twice as "akithetic").   I am sure she is trying to relate a word like "akinetic," because the admitting diagnosis was difficulty with coordination and walking, but she clearly spells this out twice as "akithetic." Has anyone else used this word before?  ...

I have an interview with Transcend on Monday and have read conflicting stories on this board about whether or not Transcend MTs run out of work.  Some you are saying you never run out; some say they hardly ever run out, and some say running out of work is a problem for them.  What's the story on this?  Also, how is the health insurance?  Thanks for any info you have!  ...
