Blank all HEENT/HENT!!! - MTinMB

Posted: Jan 06, 2015

From now on, I am blanking HEENT/HENT any time I am not 100%, absolutely, positively sure of what the doctor says.  Especially if they don't say anything at all (which is the usual).

This is utter ridiculousness and I am not going to risk a QA (or QC or whatever it is!)'s subjective opinion about whether a dictator said (or meant) one E or two!

What's next?  If they only dictate a finding on the right lung, do we have to label it as "RIGHT LUNG:" instead of "LUNGS:"?  *Maybe the doctor didn't listen to the left lung?  Maybe the patient doesn't have a left lung?*

So fed up!

The thing about this kind of stuff is... - sm

.. these doctors do NOT give a crap about that damn extra E!! Good Lord, this whole thing is so ridiculous. Those doctors do not care about that kind of stuff.

I do all straight typing still, and I have quite a few templates that the clients make themselves. They are really bad and full of incorrect punctuation, run-on sentences, and whatnot. My TSM puts it into the reports or the template database EXACTLY as it is sent from the client. I have to restrain myself from correcting some of the worst ones, but we aren't supposed to mess with it. If they don't mind that mess in their reports, they certainly don't care about HEENT vs HENT or "very well" vs "really well" <--- another one that I saw on here earlier that cost someone a whole point deduction! It's ridiculous! I thought they weren't supposed to be so nitpicky anymore. Psh! Yea right.

I can see the future ... - amy

What I was told - Sm

[ In Reply To ..]
If they say nothing, HEENT is the acceptable heading. If they mention eyes, it's HEENT because that 1st "E" is "eyes." Hope this helps.

I was told something completely different - amy

[ In Reply To ..]
I wasn't told anything about if they say nothing, but I was told that they have to specifically say two E's AND mention eyes AND ears in order for it to be HEENT.

So what happens if they do not mention the head, or the throat or--sm - anon

[ In Reply To ..]
what if instead of the throat they just say mouth? Who has time for that?

HEENT/HENT is ridiculous scoring game - E E said fast blends together - sigh

[ In Reply To ..]
Seriously? HEENT versus HENT should never be scored as there is no impact to a report either way. This has truly turned into a "who can score off the most" game that must stop as it serves no meaningful purpose. QA should focus on true patient care and client specific items that impact accurate report delivery and leave the rest alone.

From my TSM... always use HEENT - sm

From an email last week:

"For HEENT, please make sure both E̢۪s are there. Some people have put just HENT, because they think they are hearing only 1 E being dictated. This is incorrect and also a CPSE error."

What?!? That is the opposite of what QA/QC is telling us! - nm

[ In Reply To ..]
.

that is also the opposite of what my TSM - emailed us!

[ In Reply To ..]
inconsistency whichever way you turn!!

I saved the email just in case - sm

If I'm ever brought up on charges, I can use that email as exhibit A.

HEENT Vs HENT
Jan 18, 2015

I think my problem with this issue is that once you are "caught" by QA/QC and given a 1.0 deduction (by which I mean I had the universally accepted HEENT and they heard HENT, thus I was docked 1.0 for that), I now must either listen carefully to determine if they are saying HENT or HEENT, then put one or the other, then still have a chance that my reviewer will give me the error because they hear something different. If there is obviously ears and eyes that one is easy, but oftentimes it DOES s ...

Hi, I have been speaking with a company who will be hiring me for transcription.  They pay good rates and I asked them about the possiblity of job progression and eventually working as QA.  They said that QA pays by the blank... how is that even possible?  Ive seen being paid by the report, by the hour, or  by production (by the line)... but I have no idea how the QA staff make good money only getting paid for the blank.  any ideas?  Amy I missing something?  ...

Is This Blank Challenge Over Yet?
Oct 13, 2014

So sick of the emails. I really do not care what number our "team" is in, I know no one on the team anyway. If they want to reduce blanks there are better ways - talk to the dictators like they promised years ago, improve the audio, improve ASR, yada yada yada. But no. Blame the MT instead. Silly contest. All it did was encourage folks to guess in order to win a pittance gift card. Hopefully they will keep the 99.5 expectation, the only good thing that came of this supposed fiasco. ...

Look At Our Blank Winners!
Oct 17, 2014

I can pick out at least 3 in that list that I know for sure just guessed.  I've QA'd their work and it's horrible.  Way to promote accuracy MM!  You know what would get those blanks down?  Educating dicators, fixing sound quality, keeping MTs in their primaries.  Nevermind, that won't work; it's the MTs that need to improve, obviously.  MM is run by complete boneheads. ...
