Why is it okay for a QA editor to change the red-tagged headings, but not for us? We have been clearly instructed not to touch the red-tagged headings on standard work-type headings. Today I got a report back from QA and the "Procedure" section was changed to "Procedures." I know it seems like a small thing, but it irritates me. Make up your minds!

Don't be discouraged. No matter how right we are, and how carefully we follow the standards, we aren't gonna win. Once we send in a report, they can do anything they want to it, right or wrong.
One of my macros has a heading If I only have 1 diagnosis, I delete the "(ES)." Got dinged so I just leave it

I've been doing MT for over 20 years and have seen many changes. We never used to hyphenate things like "The patient is a 20-year-old. It was 20 year old. Some things did need a hyphen but not many. We hyphenate so ridiculously now that I often have nightmares of running down an abandoned street in the middle of the nigh with the boogey man shooting hyphens at me with his huge hyphen gun while my pugs dance around his feet, hoping he has some kind of midnight treat for them. (Even an obese pug can give you such a look that you actuallyh will start to believe it has not been fed in months). Aw heck. Maybe I'll catch up with the times and hyphenate my maiden and married names. Perpahs I'll start using my middle name too and put a hyphen between my first and middle names too.
In all seriousness, I have noticed over the past several months, I have gotten so much conflicting information from QA that it wasn't funny. QA are human too and they are under pressure like we are so they are going to make mistakes too. As my mom always says "There was one perfect man and they crucified him."
However, there was some serious critical stuff and one day, I sat down and made a list to send to my QATL or CCM or whoever it is who is above my direct supe. I also sent a copy to my supe. Not everything can be found in the BOS. I just wanted to know which way was correct and told her which way I was doing things. She got back to me the next day and told me everything I was doing was correct. I was surprised. I expected maybe half of my stuff would be right.
So, if you're getting a lot of info you're questioning (especially hemoglobins over 300 or conflicting information, document it along with the dictation ID and job ID and E-mail you're supervisor. I was not out to get anybody in trouble. I only wanted to make sure I was doing everything all right. Ultimately, it's up to you to make sure your documents are correct, anad ultimately, it's up to QA to make sure their corrections are correct too. As I said, we're all stressed between productivity, bad ASR, terrible American doctors (hey, if you're talking so fast that you have to say your sentence 4 times to get it to come out right, slow down!!!) and ESLs. Nobody here is assured a job if Obamacare passes. We have to make sure we are covering our behinds. Don't be afraid to ask questions about QA things you're not sure of, but be nice. Approach it with the attitude of "I'm getting conflicting info on this and want to make sure I'm doing this right." kind of attitude.
It drives me nuts to see things like you wrote - diagnoses - and there's only 1 diagnosis, and I surely would have changed it myself. So ding me. I'm old school. One doctor dictated 2 PE headings as "pulm and cards". I'd rather end up under 100% QA over something like that because I changed it to Pulmonary and Cardiac than put in what she said. I wonder if they think they're being cute.
And I will correct the grammer of these "highly educated" individuals, some of which say things like "X-ray taken today show there are a fracture of the ...". I will change the are to an is. I just refuse to put out crap. That is NOT how I was trained. If they don't like it, they can lump it.

I hate hyphens and now they put them in everywhere. Although, saying that, I also hate that they take them out where they DO belong, as in non-contributory, non-tender.
And maybe it's me, but I really despise the use of the words painfree and followup. I'm sorry, but they are NOT words!

Report it! It does seem ludicrous to complain about such a small thing, but they chose to change it and they were wrong.

Have been instructed to put the headings as dictated. (We change "Diagnosis" to "Diagnoses" if there is more than one, so it seems logical to change "Procedure" to "Procedures" if there is more than one. No ?

If it's a template we add as a standard, we do not change the headings. They are in there the way the hospital wants them, including NOT changing diagnosis to diagnoses or procedure to procedures.

Well, here's a solution I came up with and so far, QA has not called me on it (I'm sure I just jinxed myself now - ha ha)
If I have a template that is not to be changed, I just add what the doctor said to it.
Say the template says "Chief Complaint" and the doctor says "Reason for Admission."
I simply add the extra info and it comes out looking like:
I look at it this way: You didn't change the template and you also pleased the doctor by transcribing what they dictated.

It's selective! I have copied and pasted a butt load of those same types of reports to my "superiors" (using that term loosely!) demonstrating what I believe is selective corrections that vary from day to day and conflict depending on who's QA'ing I guess. I have asked over and over for clarification, justification, ANSWERS, and have yet to receive any valid response. Until they can get together and make up their minds and actually back each other up and more importantly BACK THE MT UP, we're going to continue to get this BULLSTUFF. I'm not going down without a fight tho. I am experienced and have been doing this for far too many years to sit back and take it between the eyes. ~feeling feisty.

For instance, in this dictation, she dictates lungs, describes her exam of them, then goes on to say, "Mass in the back and the left paraspinous area is measured..."
