What has happened is that with FTR, MTs are not sending anything through to QA because they don't want to get docked pay (I know that's why I don't send anything to QA anymore). The facilities are getting angry about all the blanks that we leave due to their marble-mouthed doctors, so they demand better.
We come out on the losing end like always and end up making even LESS money. The facilities need to know that the big wigs at MQ are the ones who have caused this, not us, by taking away more money from us for getting a second opinion on a report. FTR...yeah right!

Of course the doctor deserves to get a correct report, but if they don't take enough responsibility for their own patient's care by dictating clearly, then they should get a blank and let them decide what is the correct thing. I'm pretty sure that's what the OP meant..not to have that kind of responsibility placed on the MT/ME. We are penalized for sending to QA for a second set of ears now...so where else is it supposed to go? I say straight back to the client and let the dictating physician fill it in...maybe even have to listen to their own dictation and see just how crappy it really is sometimes.

[ In Reply To ..]The above poster message was crystal clear to me. We MTs are only human. MQ can't expect miracles when that is what it takes to understand those "compromised" dictations. Yet, the Q blames us for not understanding them (when we all know QA can't either) and penalizes us for not exceeding their unattainable expectations. The doctors have got to get "appropriate feedback" for reasons for the blanks we leave. MQ should stand behind US and help the doctors deliver an appropriate document, First-Time-Right, you know. Instead of dumping all the responbility and penalties on us, MQ should educate the physician about how their reports are coming through. Of course we have to give them what they want, but it just isn't being handled properly, and the above poster is as tired of it as I am.

[ In Reply To ..]I may be wrong but I thought I remembered Peter sending an email talking about MQ working on dictator standards and working with the doctors to create good dictation. Of course, he was probably just blowing smoke because apparently no one at MQ wants to stand up to the clients or doctors for us and tell those lousy dictators to that their quality is terrible. In fact, we were told to stop putting notes in the QC box about poor sound quality or background noise because the client was irritated with all the reports with notes. So once again, instead of standing up for their MTs MQ blames us and yes even penalizes us for the doctors eating/driving/going to the bathroom/answering another phone/etc while dictating. Yet another MQ promise that was broken.

[ In Reply To ..]However the client is more angry because every report with a QA note gets put into a special file at the hospital and someone has to go through and unpend all the reports. Basically we were given a list of 5 reasons to ever leave a QA note and poor sound quality is not one of them. I guess the client is sick of being told that their doctors are horrible dictators!

No we are saying if the dictation is not audible - and you have markers

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). Hi, I have been speaking with a company who will be hiring me for transcription. They pay good rates and I asked them about the possiblity of job progression and eventually working as QA. They said that QA pays by the blank... how is that even possible? Ive seen being paid by the report, by the hour, or by production (by the line)... but I have no idea how the QA staff make good money only getting paid for the blank. any ideas? So sick of the emails. I really do not care what number our "team" is in, I know no one on the team anyway. If they want to reduce blanks there are better ways - talk to the dictators like they promised years ago, improve the audio, improve ASR, yada yada yada. But no. Blame the MT instead. Silly contest. I was consistently going over the metrics for blanks being submitted to the client and decided to keep track of why I was sending those blanks. It turns out that 77% of my blanks are due to either discrepancy or audio issues; blanks that I am positive nobody could fill in. 77%! These metrics either need to be done away with or adjusted for doctors that don't take the time to not be idiots. The most common discrepancy is incorrect age or something else that I can fill ...

This just tops of my bad day. I get on to check previous reports from yesterday and see 1 correction on 1 report out of all of them. QA filled in my blank, and the error category is "major" with subcategory "not applicable." Platform is Cornerstone. Since when is it a major error to leave a blank for something we aren't sure about? From now on, I am blanking HEENT/HENT any time I am not 100%, absolutely, positively sure of what the doctor says. Especially if they don't say anything at all (which is the usual).
