Has this happened to anyone else? You cannot win for losing with this company.

What happens when you're on this list? I'm not going to guess when it comes to filling in blanks where they're mumbling or slurring or fumbling over the names of medications, etc. And if we DID guess and fill them in, we'd get in trouble with our accuracy score. So either way, I guess we're going to be on some kind of list. I'll probably be there with you soon.

I send them right through if the blanks are due to sound issues or doctor names that are not spelled or in the database as instructed all the CPs in my group.

Wonder how the battle of the blanks is working out. LOL. Nothing has changed in the number of blanks I send straight to the client, and nothing will change until they fix the real problem, sound quality and doctor education regarding how the dictate.

I just could not make out what the client was dictating due to poor sound quality. In that case I figured if I could not make out what was being dictated that sending to QA would not help. One reason I did not send to QA was because our team received an e-mail to limit the number of reports that are being sent to QA. No matter what we do it ends up being wrong.

An an MME, many MTs send to qa cause they say they cant hear but - I can the audio just fine. nm

Maybe your hearing is better?

Is it possible that sound varies from computer to computer?

Maybe you are more familiar with the word or phrase so it is clear to you and may not be to someone who hasn't encountered the word previously?

From personal experience, sometimes the dictation sounds like gibberish at first, and then upon repeated listening it finally comes to me and then I wonder why I strugged so much with it.

Often when I go back over blanks when I've finished typing the whole report, the dictation seems to be very clear, although on the first listen, I had no idea.

Curious about MMEs: Do they send reports to QA? Are their blanks filled in?

Just want to add to this that sometimes an MT may send to QA just for confirmation that what they hear is correct. Maybe they are not familiar with the phrase.

can resolve the issue. But I figure since I am the last line of defense against issues, if I cant resolve it, likely nobody else can either. So reports I do go straight through and if it is an issue I cant fix, I leave a qc note to the facility knows there is a problem. Excess blanks for those horrible dictators, just go through with no note. They know who is a horrible dictator and I am sure are not surprised when those reports have excess blanks.

The reasons you give for MTs not hearing on a report are good ones. I just figure the MTs don't want to take the extra time to figure out a difficult report and send it to qa so they can get onto the next job.

Just like MTs, our report go through qc audits. So we cannot just send reports to qa for silly stuff, it has to be a real problem that cannot be resolved by us.

I would like to add that there have been times that I have heard a word very clearly, but in the end it turns out to be something else (like a sound-alike). I feel that if I can hear it, that doesn't mean I am correct unless I can find it on google.
Yep, I fill in lots of "inaudible" blanks. That's why we're here, so let us - at least try. NM

I understand they need to somehow govern how much work is being sent to QA because there have always been a few MTs who would send a blank to QA rather than research it, probably not even realizing how many they are sending, but I have always felt the percentage you are allowed to send to QA before being docked is too small.

Either the percentage needs to be raised, or if QA is unable to fill it in, then that report should not count against your overall percentage of lines sent to QA (I was told by my HDSM that it does, regardless of whether or not QA was able to fill in the blank).

It has always been my feeling that if you focus too closely on the bottom line, quality is going to suffer; there just isn't any way around it, and blanks to the client are one of those ways.

There have been many, many occasions where I have HAD to send to QA, and even QA could not fill in the blanks on some of these dictators.

We should not have pay docked if QA cannot fill in blanks either. I am grateful for QA, but with some of these dictators, they cannot fill in the blanks either.

mumbling docs or those with thick accents can make you either pull your hair out by the roots or you will have to guess - not good- or leave a blank for the dictator to hopefully fill in - not good either.

Are you putting underscores instead of QA markers? - Always have to use QA markers. NM

This was posted a few months ago and I kept a copy of it. I truly feel it says it all!

Shame on us for sending something to QA because the dictator didn't enunciate.
Shame on us for leaving a blank for the words cut off while using their pause command.
Shame on us for allowing the doctor to input the wrong MRM.
Shame on us for questioning dosage of Hydrocodone, 1,000 mg.
How dare we alert them that the patient is allergic to a prescribed med.
How dare we alert them to an incomplete dictation
How stupid of us to not know exactly what medication was covered by a cough.
In fact we are soooo stupid that we are the ONLY one having these problems.
Here is the unwritten MTSO Policy Manual: Just fill in a blank with whatever they probably won't catch. Donate as much of our time as it takes to correct demographic errors or be soley blamed for same. Let the patient go into anaphylactic shock. Who cares. Don't tell them their dictation is not complete, it is damaging to their sensitive egos. Other docs can just guess what the plan was and newly prescribed meds. When you don't understand a medication, DON'T burden QA with such nonsense, just fill in some other drug it might be. Never tell a doc his dosage was wrong, as again, we would rather kill the patient than hurt his sensitive ego. Finally, we need to just accept the fact that we are their stupidest employee or contractor. Only we can screw up as much as we do. We are worthless.

you can send a QA marker with a note directly to the hospital, what they are talking about is sending _______ to the hospital. That you can never do, it always has to be either Q/A with note directly to the hospital or QA to the QA Dept.

I'm just sayin....as a radiology MT in this company for 14 years...we have always sent radiology reports straight through to the docs to review and sign (because of turnaround time)....never knew they did QA until I got new accounts 2 years ago....and have had some pretty bad foreigners.  I believe it is docs job to go over his reports anyway...he is legally responsible.. ...

just shows which people are getting the gravy work while I'm drowning in the muck.  The people sharing the nightmare accounts I'm on will NEVER be able to have zero blanks or even a perfect score considering the putrid garbage we're trying to hear and interpret, while obviously there's a nice little list of lucky people getting things they can hear & I'm sure able to make a lot more money as well.  So glad they decided to rub that in to the rest of us!  ...


Short Cut For List Punctuation In A Drug List? (Fluency)

Is there is quick way to add commas between items in a list? Of course, using Ctl K in front a word will change the punctuation, but it will also change a capitalized drug to lower case, and you will still have to get to the beginning of the next drug which will require 2 Ctl arrows and backspace. Ctl arrow takes you to the beginning of the next word, but you will have to backspace to put in the comma. Adding punctuation to lists is so time consuming! ...

Just wondering about the sending of blanks.  I had a few blanks due to inaudible dictation.  I know QA wouldn't be able to get it either, but when I uploaded the job the QA box was checked and I tried to uncheck it and it wouldn't let me.  So, how do you just send it straight to the client and not to QA? ...

Are they kidding?  Who is going to be sending ANYTHING to QA and take a chance of having their pay cut? Especially if it's the beginning of the work week! Yes, the clients will be getting blanks and I know they will be complaining.  The Q keeps sending us e-mails telling us to cut down on QA submissions. Are they kidding? No amount of "tips" will help if the dictator mumbles, sneezes, is ESL, etc. Don't they think we know all the "tricks" already? Besides that, we can't ...

I feel that I flag a lot and send to QA a lot.  I've been at this for about a year, do acute care, lots of ESL etc so I think it's a reasonably difficult account; but I ask myself do really experienced MTs, e.g. those at this for 5 years or 10 years or more, send so many jobs over to QA?  I mean, do you get to the point where you can pretty much understand everything these guys are saying, even when they're chewing, slurring, belching etc.?  I would love to make it ...

I have just had it.  I was switched over to a horrid account, and every time I ask about being switched back, I am told to just keep on working.  The account I was switched to have horrid AMERICAN doctors as well as ESLs.  I have sat here for months listening and re-listening to no end.  We are always sent emails not to guess, send everyting to QA, well I have just started too.  No longer am I sitting here trying to guess, only to have to send the report to QA anyway.&nb ...

Utterly confused. I took a big, BIG hit with the QA deduction in paycheck this week. So, I decided to investigate. Like everyone else, I have about 10 "regular" accounts. No blank rule is mentioned anywhere on any of the Account Specifics. I wrote the QA person for my CMC questioning how many blanks were acceptable without sending to QA, and got the "first time right" explanation. This is BOGUS...if I have one blank on a lengthy report and hafta send to QA and they take the total line ...

I got a complaint from corporate that my lph are not where they should be via my TSM. I have been at MModal 6 weeks and just got off training. I was hired in at tier 3. My lph are gradually going up every day but not to suit corporate. I do straight transcription, no VR, on Futurenet. A lot of ESL bad dictators. I am still getting new dictators every day I work. I am working on sending back my equipment for MQ and just got the mailing slips in the mail today.  The sheet that comes along with it tells me I'm not required to send back the foot pedal, speaker, keyboard or mouse.  This makes no sense to me.....anyone been here before??  Did you just send everything back regardless??  Please help. ...

Okay, I got a ding today from my extremely sweet and helpful QATL.  It seems I did not send as a cc a requesting physician dictated.  I looked at the report and I did enter the requesting physician in the ADT and the body of the report.  I thought if they did not specifically request a cc be sent, then you did not.   Am I wrong?   Old dog needs help.  Also, I thought if the Req Physician was in the ADT it automatically received a carbon copy..  Hmmm.. ...
