I'm seeing a few ads for transcriptionists/editors to work directly on the facilities EMS system. I'm wondering if this might be a new trend.
It's all about saving money and turnaround. When it comes to transcription, I think the MT working directly for the client was the most cost effective. VR failed as they realized a brain was needed to do the necessary editing & formatting. The MTSO takeover reduced us to slave wages, but they are probably still charging line rates we used to make. This bought the facilities the benefit of fast turnaround.
Now I'm seeing hospitals advertise for MT/MEs to work directly on their EMS system. Can cutting the middleman bring them further savings while maintaining fast turnaround or will they need to overstaff or have per diems to cover overflow the way MTSOs did to maintain turnaround?
Will we see the demise of these MTSOs while opportunities open up to work directly for the clients?

[ In Reply To ..]They have given me log-on access to it just as you would have when working for a MTSO with passwords, etc. I would think it would work the same way for a large facility. The office staff/hospital fills in a good deal of the information on site. The doctor dictates those narrative portions that he feels point and click does not cover. Unfortunately, the EMR's medical dictionary is just awful and my text expander does not work with it. I think most of the EMRs are NOT designed to be transcription friendly. I also cannot count my lines in the finished document because it contains so much other stuff that was not actually dictated. What I do is transcribe it in a word document so I can count lines, then I copy and paste it into the various sections of the EMR where he dictated. I've been doing this for over 3 years. I'm sure it could be done on a larger scale. The EMR setup is not that hard to use either. The office manager showed me once which sections I would be transcribing in. I would not be intimated at all with transcribng in an EMR as there is probably going to be a lot more work like this coming along. If you have any more questions, please post.

[ In Reply To ..]While I think most of us will be out of work, I do think it makes good business sense to use MT/MEs directly, eliminating the MTSO, giving us better pay, and saving them money on lines, with fewer lines necessary due to point & click. I think methods for creation of medical records is still in transition.
Does the dictionary used with your EMS learn words?

[ In Reply To ..]I use my Stedman's when I transcribe the notes and don't generally use their spellcheck function. I did it once for kicks, and that's when I found out it did not know the majority of the words in the document. I didn't check to see if they could be added. I really did not want to alter the program too much as I'm not the only one who uses it.
I'm not sure I fully understand/agree with your concept of using MTS/MEs directly. It would mean health care facilities/clinics would have to enter into contracts with individuals and or hire individual employees. That would be a lot of work and $$ for them to pull it off.

[ In Reply To ..]My last in-house job was for a hospital where we had a transcription department that consisted of about 60 full time employees and about 20 ICs working from home. The hospital had a standard contract and paid the ICs hourly and everything stayed within TAT. After I left they outsourced to an MTSO but I always wondered why they didn't just give the employees the option of working at home as ICs as it would have saved them money.
Another question I'd like to ask anyone who might know is if all EMRs have the capability to send sound files and receive the transcription directly into the chart from a remote MT/ME.

[ In Reply To ..]I don't believe many of the ones designed for smaller practices are MT ready for uploading and downloading voice files and sending the work back. No, they do not use EPIC. There are only 4 physicians, and I believe EPIC would be quite expensive for such a small group. They use EMR4MD. It does not have the ability to send sound files. I actually pick up 2 tapes once a week from them. He only sees patients 2 mornings a week (10-20 patients per week). I looked into a secure FTP program where he could dictate on a digital recorder and upload to my site, and I'd pick it up from there; BUT the cost of that per month for me and for him to buy a digital recorder didn't make sense for the small amount of work. Anyway, I enjoy getting out of the house once a week and going for a drive. If I had 3 or more physician offices that I transcribed into their EMR, I'd certainly do that though. There are lots of EMR programs for small group practices out there. Just do a search to find out what they are and to see which ones might be more transcriptionist friendly for voice files, etc.

They went over to EMR but we are also allowed into the transcription module to cut and paste our notes. Bit more work having to cut and paste but it is do-able. Recently was asked by another group to do the same thing for them.

[ In Reply To ..]Before they had the capability for me to transcribe onto their system, I typed in Word and they cut and pasted it into their EMR. Then they had some platform that I was able to type into using Go To My PC. Then when they got ExText and ExEditor, I could just log in and type into their system. I have been doing it for 15 years.

beyond repair. We let it go on far too long, the abuses. The lawsuit will help, but it is not enough, and it will take too long. Somehow I have gotten this virus on my laptop and when searching for ways to remove it on the internet (on my desktop), all the sites are either extremely comlicated or want you to download things in order to remove it. My New Grading System

It's simply unfair that MQ pays me the same same whether I do a normally dictated report, or one that is handicapped. Handicapped reports obviously take much more time to finish.
