Transcriptionist's responsibility RE: Missing dictation - gracie64

Situation - Clinician does not dictate on a patient for a DOS or (even worse) routinely does not dictate their sessions with patients.  Is it the transcriber's responsibility to go though clinicians' schedule daily and check to see that there are corresponding voice files to patients seen and, if not, e-mail reminders to clinician to dictate.  I feel like I have to take some of them by the hand and remind them of something that is so obvious to their job. We are talking about people with Masters degrees and above.  

Or, should this responsibility fall on support staff of the department?

Has this responsibility been assigned to you or have you taken it upon yourself to do this? It really sounds like a job for the office manager and/or billing person (who may be one in the same). After all, if there is no documentation of a patient's visit, there can be no billing generated. If it is someone else's job, can you ask to be relieved of this duty so you can do what you do, transcribe? You might just be able to give the office manager a list of patient names who have missing dictation and let that person take care of the reminders to the clinician.

I used to have an account where I would get phone calls and emails all the time about why I did not send in a report on this patient or that. It drove me batty. I am in one state and the office was in another. I started sending a list of patients with each batch of returned work (one batch returned for each date of service). I would still get the calls. Now keep in mind, the doctor paid me (he was an IC like me and just provided services at that office) not the office. I finally sent a formal request for a $25 fee for each missing dictation request to the person in charge with an explanation that my obligation was to the person who paid me and did not include researching lost reports for them.

The requests stopped immediately because they began getting the doctor to initial each chart after he completed his dictation. Part of their problem was that he would see the patient and then hand off the chart to follow the patient to check out. He would usually get the chart back, but on occasion it would go on to someone else and he would not get it back that day. Since he only worked there one day a week, it would be a delay of a week to get the chart back to him.

Maybe if you have a conversation about how the flow in the office works so you can pin down the problem, you could offer suggestions to them (such as initialing the chart "just until we figure out where the problem is" or "so I don't have to add an administrative fee to research lost reports").

Unless it has been specified that you are to provide that service, you shouldn't. Unless you are being paid to provide that service, you shouldn't. Remember that even people with master degrees can be incredibly busy and they should be smart enough to hire support staff to keep them organized and on track to maintain a successful business.

I am not on site, but telecommute. My pay is calculated by line count only. Hours aren't calculated into my pay.

It is my responsibility at facilities I transcribe for; however, I charge for the list now. It is not my sole responsibility; we all check.

Before I waste even more time sending a useless ticket to support...I would assume (and yes, I know what they say about assume :))  that someone has already sent a ticket for this, just wondering what kind of explanation, if any, did you get and are they considering fixing it?  It is getting so old doing the ctl+H to find the missing dictation, then backspacing to get rid of the period and putting it where it truly belongs..then doing ctl+H on correct verbage, then changing upper ...

I got in trouble today because I put a wrong patient.  The doctor dictated a billing number, name, and MRN.  The MRN was had a different patient, but the billing number and the patient name matched the ADT screen.  Not to mention the fact that he kept using "she" in the dictation and the MRN number matched a "he".  So logically one would just think he had the wrong MRN and I didn't want another QA submission so I sent it through with the correct billing number, name, and ...


Thoughts About Our Responsibility For Dosages

I have been trying to get another job being an MT that is either FT or is PT to supplement my already PT job.  I have sent out way more resumes that I can count and only got one job offer.  This job offer was for 5 to 6 cpl.  I could have worked full time on this job, but I feel that an MT is worth more than what they were offering.  My questions are am I missing something?  Am I doing something wrong?  What else can I do?  It's just frustrating to finally ...

Missing HPs When Typing DSs?

Okay, I have had the suspicion that MQ programmed something when you bring up old reports when typing a discharge summary that the H&P doesn't show up.  Today, the dictator doing the DS plainly stated that she was reading right off the H&P.  I went to check in the old reports to maybe get some quick lines, but it wasn't there...as usual.  I typed what she said and realized that I had already typed this before and that I was the one that typed the original H&P ...

When you suspect that a word may have been omitted from dictation, do you make a note for this to be reviewed or do you think that is overstepping your bounds and basically saying the doctor made a mistake?  For instance, this ESL doc said "Bacterial endocarditis is not particularly recommended, but the patient has had Augmentin or amoxicillin and may be given a single dose.."  I suspect he forgot to say bacterial endocarditis "prophylaxis" is not particularly recommended...etc.  ...

Missing Keystrokes

I am a remote transcriptionist for a large hospital.  We recently went live with VR Nuance/escript.  Ever since go live I have had the continuing problem of typing a voice file and will lose a keystroke, have to backspace, sometimes hit the same key 3-5 times before it will show up on the screen, sometimes taking me an hour to 2 finger type a report.  This has been happening since the first day of go live.  IT, Nuance nor escript have any idea what is going on.  Has this ever happened to an ...

MQ uses Instant Text for their line counts.  I knew i had it for word expanders but prefer to use my own WEs.  Well, they have a line counter too but I haven't had the time to check it out. So, today, I took about 5 documents, copied and pasted them into Word.  MQ requires 65 CPL, Word requires 73, so I should have come out behind in Word each time.  Instead, I came out behind in every document, the least being amount 11 lines and the most was 29.  T ...

Yep, no accuracy percentage, if you worked for them you were good and they knew it, not need to prove yourself. No minimum line counts per hour, work and do your daily minimum whenever at your pace, as long as it got done. No dings for sending to QA or asking for clarification.  No outrageous taxes taken out of bonus, earned $400 got $400.  Sure do miss those days when we were treated as adults who could be trusted to do a good job.  ...
