No kidding. Why on earth don't doctors know how to pronounce the medicines they prescribe?! I get "alpralazam" also and "donezepil" and so many others. Some of them know they can't say the words, but I find out they can't spell it either. When they do spell it right after saying it wrong, it's funny... I'm waiting for the "oops, my bad on pronunciation".

I got a really good laugh at one doc who pronounced omeprazole like "oh mee pra zole eee" like it was a pasta. I can understand a drug that's new or one that is used in another specialty that they are not familiar with but the common ones are messed up just as badly !

I find the nurse practitioners to be the worst for mispronouncing drugs, but they're all bad at times. I hate it when I'm pretty sure I know what they mean, but have to leave it out because I can't take a chance and be wrong.

I get so tired of these people slurring everything together, speaking so fast because it's what they always say.  Then we're told by QA, they the same thing over again so you should know what it is.  If I could understand it, I wouldnt leave a blank!!!   We're told not to guess.   Especially lab values or other numbers or values.  No wonder we leave blanks, when we can't tell if it's  180 or 190 because you're speed slurr ...

I started a second job for extra money and so I'm on 100% QA.  I keep getting feedback for what I consider to be the most inane reasons.  I am old school.  When a dictator starts as sentence "patient is here for..."  I type "The patient is here for ...."  So QA has pinged me several times for not typing "verbatim."  So I've tried to be very conscious and listen very closely.  Now I've gotten feedback where I clearly thought I heard patient, typed ...

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested."  I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..."  So I decided to omit crappy and just say "she is still sounding pretty congested."  QA w ...

How many of you leave blanks when medications are not pronounced correctly? Or fill in because you are sure of what medication it is supposed to be? I've seen QC and QA fill in the medication when it was badly mispronounced. Example: Methylprednisone for methylprednisolone (there is no methylprednisone).   ...

Moved from Hawaii to Florida and thinking about going back to IC status.  Been doing some research on the state tax law as I heard/read FL is 1 of 7 states who exempts sole proprieterships from filing 1099 and even corporate income tax.  If my understanding is correct, wondering if I should chance IC again, even though it seems MTSOs haven't changed as far as putting you on a schedule :(.  Ideas anyone?  TIA. ...

I have a great part-time IC job that I really love. It is so stress-free and I only hear from my account manager when she sends me my pay summary right before pay day. I have been thinking about how blessed I am to have found them especially after working as an employee for a couple other MTSOs. My employee jobs have been so stressful and QA can be so petty with a lot of these places. A week and a half of this and not getting better.  Emails about new quality standards and as I will be transitioning into fluency I will be making below minimum wage which is not going to cut it for my bills.  How is this legal???  No work during shift, complicated procedures which take up MY unpaid time to try and figure out, unrealistic standards when there is work, having to meet an hourly production minimum but get a massive paycut if you need help.  UGH! This is almost/sort of a poll. I need to know what verbatim means to you in VR. Do you change any wording? My understanding of verbatim is word-for-word (Merriam-Webster dictionary) unless a sentence does not have proper structure; if the doctor rambles on and puts the word 'and' between words where there should be a period, etc.  (Not included in this is changing mg to mcg in a wrong dosage, changing the medication itself if wrong, or other major medical erros&nbs ...

I have all verbatim-verbatim accounts and only just now learned that we have to change what the dictator says and change it to an "approved heading."  I was once told this only pertained to non-verbatim accounts, but was just corrected.  I was told that ALL OF THE CHECKED headings in the headings pane must be included in the document.  If the doctor says procedure, you must put in an approved heading into the standard changing what he says if in the approved headings list it says ...

clearly dictates a sentence that makes absolutely no sense.  Just sort of an open-ended sentence, not even a fragment?  I'm trying so hard to keep the blanks down, which my % is really okay, but I'm finding more and more that these docs are just not making any sense and the mistakes are getting ridiculous!!!  Maybe they are texting when they are dictating........Anyhow, do you just type what they say and send it off or do you mark it for QA? ...
