A patient came in to the office, threw his urine drug screen req form at me, spouted off profanities at my doctor, got in his face, threatened him with both physical violence and a lawsuit (for telling him to leave and calm down and then come back), called him a bunch of names and then stomped off.  Can't bill for that, can I?  It seems a shame since that slot could have been used for a good patient, not to mention the whole thing took 25 minutees.

Who is paying for it?

Employer? Prospective employer? Bill them. Be sure to explain exactly what happened.

Parole office? Bill them. Be sure to explain what happened.

Drug rehab place? Bill them. Explain the behavior.

Insurance company? Sure, I would bill them. Explain the lost office time.

Medicare? Probably not.

The patient? Definitely NOT! Too dangerous.

Don't give him another appointment. If he shows up again like that, call the police. You should have called them after the first 5 minutes last time.

Just because he didn't make it to a treatment room doesn't mean he wasn't seen. Certainly sounds like some counseling happened.

Don't use 99211, either--this necessitated the physician's participation. Go with the "counseling and coordination of care" provision. Bill for the new or established patient E&M that is closest. It is between 99202 (20) and 99203 (30), or 99214 (25).

Definitely establish a record on this patient. Have the physician document the encounter.

I have a question ?? I am a transcriptionist in a surgeons office.  I transcribe the report, fax it if it needs to be faxed and it gets filed in chart.  The billing takes place 2 to 3 weeks later for the service depending on how current the coder is.  Now.... this is the question.  The physician is very greedy and likes to have level III and IV charges.  His gait keeper has decided that he was not dictating enough and not getting the higher levels and too many level IIs. ...

I swear to all that is holy, this is what this less-than-stellar PA just dictated:   "The patient IS a high-risk gestational trophoblastic disease..."  I went back and listened thinking I must have heard wrong and she said patient HAS ... Nope.  She said IS.  This is the same PA who dictated a creatinine of 92 for someone.   That sound you hear is me hitting my head on the desk. ...
